Application Procedure

The applicant is required to be present at the NRD counter at the headquarters in Putrajaya together with supporting documents:

• Identity card

• Original marriage certificate

• Letter of declaration of oath

• Other supporting documents (if necessary)

Conditions of Application

• Fill up JPN.KC08 application form and attach supporting documents.

Payment:

• Processing of clerical errors: RM2.00

• Processing of Factual errors: RM2.00

• Application approved: RM5.00
