Identity Card of Children below 12 Years

 

CONDITIONS OF APPLICATION

  • The applicant must be 12 years old but has not attained the age of 16 years.
  • The applicant must be present together with the Sponsor (Mother or Father of Guardian).
  • Application Form JPN.KP01 (available at NRD counters) has been completed.
  • The applicant must make the application within 30 days from the date the applicant attained the age of 12 years. Otherwise, a compound and a processing fee will be imposed.

 

APPLICATION PROCEDURES
The applicant is required to go to the NRD Counter with the Sponsor and bringing along the following supporting documents (original) :

  • Birth Certificate/ Adoption Certificate/ Citizenship Certificate/ Confirmation of Citizenship Status/ Citizenship Form/ Appendix A (JHEOA) / Entry Permit.
  • Identity Card of Sponsor, i.e. Mother or Father or Guardian.
  • Statutory Declaration (for Guardian without any family relationship, i.e. other than mother/father/grandfather/grandmother/uncle or aunty or sister or brother who is above 21 years old).
  • Passport (for citizen who has just arrived from abroad/ holder of Entry Permit).
