CONDITIONS OF APPLICATION

  • The applicant has attained the age of 12 but not 16
  • The applicant must be accompanied by a sponsor (mother or father or guardian).
  • Application Form JPN.KP01 (available at any NRD counter) must be completed fully.
  • The applicant must make the application within 30 days of the date the applicant attained the age of 12. Otherwise, a fine and processing fee will be imposed.

 

APPLICATION PROCEDURES
The applicant is required to go to any NRD counter with the sponsor, bringing along the following documents (original):

  • Birth certificate/adoption certificate/citizenship certificate/confirmation of citizenship status/citizenship form/Appendix A (JHEOA)/entry permit
  • Identity card of sponsor, ie. mother or father or guardian
  • Statutory declaration (for a guardian with no blood ties, ie. other than the mother/father/grandfather/grandmother/aunt or uncle/sister or brother who is above 21 years)
  • Passport (for citizens who have recently arrived from abroad/entry permit holders)

APPLICATION REQUIREMENTS

  • The applicant is 16 or older.
  • The applicant must be accompanied by a sponsor (mother or father or guardian).
  • The applicant has never applied for or owned an identity card and resided in Malaysia from the age of 12 to 16.
  • The following forms must be fully completed (available at any NRD counter):
    (i)   Application Form JPN.KP01
    (ii)  Statutory Declaration (Form JPN DL-1)
    (iii) Explanatory Form for Late Registration of Identity Card by Mother/Father/Guardian (Form JPN DL-2)
    (iv) Extract of  School Register (Form JPN D L-3); or
    (v)  Declaration for Children who have never go to school (Form JPN DL-4)
    (vi) Family Information Form (BMK.81 & BMK.81A)

APPLICATION REQUIREMENTS

APPLICATION PROCEDURE
The applicant is required to go to any NRD counter without any documents or with the following supporting documents, if any (the original and a copy):

  • Attestation letter or Welfare Card from the Department of Social Welfare /attestation letter  from the Department of Orang Asli Affairs/headman/village chief/chieftain/district office/ chairman of the Village Working Committee - for the poor and destitute, and disabled persons
  • Attestation letter from the Department of Social Welfare/Fire and Rescue Department/headman/village chief/chieftain/district office/chairman of the Village Working Committee - for those affected by natural disasters
  • Attestation letter from prison/drug rehabilitation centre - for former prisoners or centre trainees
  • Student card or attestation letter from the school principal/registrar of institution of higher learning - for students from any local or foreign school, college or institution of higher learning
  • Police report stating the full details of the incident and lost identity card

If an application for MyKad replacement is made within 12 months of the date of handover of the card, no fee will be imposed (FREE).

If an application for MyPR replacement is made within 12 months of the date of handover of the card, no fee will be imposed (FREE).

Note: According to Regulation 13 of the National Registration Regulations 1990 (Amendment 2001), every person whose identity card is damaged or destroyed must make an immediate application for its replacement.

 

APPLICATION REQUIREMENTS

  • Applicants must be present in person.
  • Change of Address Form (available at any NRD counter) must be fully completed.
  • The address must be a permanent address in Malaysia. The address of a workplace, office, organisation or PO box will not be accepted.

Note: According to Regulation 15 of the National Registration Regulations 1990 (Amendment 2001), every person must apply for a replacement of identity card on a change of address where she/he will reside for a period of 90 days or more.

    *Sponsor: parent or guardian.
  • The applicant must be present in person.

  • The applicant must make the application within 30 days of the date of reference in the entry permit (oval stamp). Otherwise, a fine and a processing fee will be imposed.
  • The application can be made only at the Putrajaya NRD Headquarters and NRD State Headquarters.

This involves the process of altering or amending information in the identity card such as name, date of birth and religion.

APPLICATION REQUIREMENTS

  • The applicant must be present in person.
  • The applicant must possess an identity card. Applicants who have yet to own an identity card is not eligible to apply.
  • Application Form JPN.KP16 (available at any NRD counter) must be fully completed.
     

APPLICATION PROCEDURE
The applicant is required to go to any NRD counter, bringing along the following documents (the original and a copy):

  • Statutory declaration (if the applicant is below 18, the declaration shall be made by the mother/father/guardian)
  • Identity Card.
  • Birth certificate/adoption certificate/certificate of citizenship/ confirmation of citizenship status/citizenship form/entry permit (whichever is applicable)
  • A copy of identification document of the mother/father/guardian for applicants below 18 years of age (copy of identity card or temporary identification document)
  • Passport (if any).
  • Other supporting documents relevant to the application, such as:
  •    a.    Card declaring embracing of Islam for those who have embraced to Islam with or without change to the original name.
       b.    Shariah court order/attestation letter from the Islamic Religious Council or Department for the application to convert from Islam to another religion, and to change from a Muslim to a non-Muslim name.
       c.    Baptism certificate/attestation letter on embracing of Buddhism/Hinduism/Sikhism for religion change application affecting name. If the original name is maintained, then only Appendix “A” needs to be filled at the counter and the above supporting documents will not be required.
       d.    Marriage certificate KC07 issued by the NRD for the application to enter the husband’s name for those married to foreign citizens only.
       e.    Other documents required by the department to support the application to alter particulars.

1.    Citizens/Non-Citizens = RM10.00 (processing fee for alteration of particulars)

If the application for alteration of particulars is approved, a replacement card application fee will be charged as follows:
a.    Citizens = RM10.00
b.    Non-citizens = RM40.00

Note: According to Regulations 14 (1) & (2) of the National Registration Regulations 1990 (Amendment 2001), every person who applies to change his/her name or to amend the particulars in the identity card other than the address must immediately apply for a replacement identity card.
