APPLICATION FOR IDENTITY CARD (MYKAD) FOR MALAYSIAN CHILDREN AGED 12

 
1. REASON FOR APPLICATION
1.1 Regulation 3 of the National Registration Regulations 1990 (Amendment 2007) states that a Malaysian child who has reached the age of 12 must register for the identity card.
 
2. APPLICATION CONDITIONS
2.1 The applicant must have attained 12 years of age but is below 16.
2.2 The applicant’s sponsor must be present, who is either the 
2.2.1 Parent; or
2.2.2 Guardian (the guardian could be the grandfather, grandmother, aunt, uncle, elder sister or brother who is over 21).
2.3 Application Form JPN.KP01 (obtainable from the NRD counter) that is completed.
2.4 The applicant must submit the application within 30 days of attaining the age of 12. Otherwise, a processing fee will be charged.
 
3. APPLICATION PROCEDURE
3.1 The applicant and the sponsor are required to go to any NRD counter with the necessary original documents.

LATE REGISTRATION OF IDENTITY CARD (MYKAD OR MYPR)

 
1. REASON FOR APPLICATION
1.1 Applicants aged 16 and above who have not been a bearer of the identity card and have resided in Malaysia throughout the period of time must apply for late registration of identity card.
 
2. APPLICATION CONDITIONS
2.1 The applicant must be aged 16 and above.
2.2 The applicant must be accompanied by a sponsor, who is either the 
2.2.1 Parent; or
2.2.2 Guardian (the guardian could be the grandfather, grandmother, aunt, uncle, elder sister or brother who is over 21).
2.3 The applicant has never applied for and has not been a bearer of the identity card and has resided in Malaysia from age 12 to 16.
2.4 The following forms (obtainable from the NRD counter) must be completed:                                                                      
2.4.1 Application Form JPN.KP01;                                                                                  
2.4.2 Statutory declaration (Form JPN DL-1);                                                     
2.4.3 Explanatory form for late registration of identity card by the parent or guardian (Form JPN DL-2);                                                         
2.4.4 Extract of school register (Form JPN DL-3);                                                                       
2.4.5 Statutory declaration for children who have never been schooled (Form JPN DL-4);                                                               
2.4.6 Family Information Form (BMK.81 & BMK.81A); and
2.4.7 Form RK-JPN-KP-PU-06-03 (for sponsors other than those allowed)
 
3. APPLICATION PROCEDURE
3.1 The applicant and the sponsor are required to go to any NRD counter with the necessary documents.

APPLICATION FOR IDENTITY CARD (MyKAD OR MyPR) REPLACEMENT FOR CITIZENS AND NON-CITIZENS AGED 18 

 
1. REASON FOR APPLICATION 
1.1 Regulation 18 of the National Registration Regulations 1990 (Amendment 2007) states that any person who has attained the age of 18 and is a citizen or permanent resident identity card bearer must have a replacement identity card made no later than the age of 25.

2.1 The applicant must be aged 18 to 25 years.
2.2 The applicant is required to go to any NRD counter.

APPLICATION FOR REPLACEMENT OF LOST IDENTITY CARD (MYKAD OR MYPR)
 
1. REASON FOR APPLICATION
1.1 Regulation 13 of the National Registration Regulations 1990 (Amendment 2007) states that a person who has lost his/her identity card shall immediately apply for a replacement.
 
2. APPLICATION CONDITIONS
2.1 The applicant is required to go to any NRD counter; or
2.2 The applicant can apply for replacement of lost identity card online at www.jpn.gov.my using the "MyKad Replacement System" application if no change of photograph or particulars of the Malaysian applicant is involved.  
 
3. APPLICATION PROCEDURE
3.1 The applicant is required to go to any NRD counter. No documents are required but supporting documents may be presented (originals and copies).
 
4. SUPPORTING DOCUMENTS (ORIGINALS AND COPIES)
4.1 Certification letter or Welfare Card from the Department of Social Welfare, or certification letter from the Department of Orang Asli Development, a village headman or chief, or the district office or chairman of the Village Development and Security Committee (for the poor, needy or disabled persons); or
4.2 Certification letter from the Department of Social Welfare or the Fire and Rescue Department or a village headman or chief or the district office or chairman of the Village Development and Security Committee (for those impacted by natural disasters); or
4.3 Certification letter from the prison or rehabilitation centre (for ex-prisoners or centre trainees); or
4.4 Student card or certification letter from the school principal or the registrar of institution of higher learning [for students from any school, college or institution of higher learning both local and foreign]; or
4.5 Lost identity cards resulting from burglary, robbery, theft, catastrophe and the like require submission of a police report clearly stating details of the incident and the lost identity card.

1. REASON FOR APPLICATION 
1.1 Regulation 13 of the National Registration Regulations 1990 (Amendment 2007) states that any person whose identity card is damaged or destroyed shall immediately apply for a replacement identity card.
 
2. APPLICATION CONDITIONS
2.1 The applicant is required to go to any NRD counter; or
2.2 Apply online as follows:
2.2.1 Through the website www.jpn.gov.my using the “MyKad Replacement System”; or
2.2.2 Phone-in service at 03-88808066
(valid for Malaysian applicants who do not need to change their photographs or particulars)
 
3. APPLICATION PROCEDURE
3.1 The applicant is required to go to any NRD counter with the required document (original).
 
4. REQUIRED DOCUMENT (ORIGINAL)

5.3 Identity cards damaged within 12 months of the date of issue will be replaced WITHOUT CHARGE.
5.4 Payment for website applications to replace damaged identity cards can be made only with credit cards.
5.5 Refunds will not be considered for damaged identity card applications that have been processed by the Identity Card Division.

APPLICATION TO CHANGE IDENTITY CARD ADDRESS
 
1. REASON FOR APPLICATION
1.1 Regulation 13 of the National Registration Regulations 1990 (Amendment 2007) states that the holder of an identity card who will be changing the current place of residence to another place of residence for a period of 90 days or more shall change the address on the identity card.
 
2. APPLICATION CONDITIONS
2.1 The applicant is required to go to any NRD counter.
2.2 The Change of Address Form (obtainable from the NRD counter) that is completed.
2.3 The provided address must be a permanent residential address in Malaysia. Addresses of the workplace, office, organisation or post office (PO) box are not allowed.

1. REASON FOR APPLICATION
Regulation 3 of the National Registration Regulations 1990 (Amendment 2007) states that a citizen who was not resident in Malaysia from age 12 to 16 must register for the identity card within 30 days of the date of arrival in Malaysia.
 
2. APPLICATION CONDITIONS
2.1 The applicant must be aged 16 or above.
2.2 The applicant is required to go to any NRD counter.
2.3 The applicant was not residing in Malaysia from age 12 to 16 (passport used as proof of travel).
2.4 Application Form JPN.KP01 (obtainable from the NRD counter) must be completed.                                         
2.5 The application must be made by the applicant within 30 days of the date of arrival in Malaysia. Otherwise, a processing fee will be imposed.

1. REASON FOR APPLICATION
1.1 Regulation 3 of the National Registration Regulations 1990 (Amendment 2007) states that a permanent resident is required to register for the identity card within 30 days of attaining age 12, or of the reference date of the entry permit issued by the Immigration Department of Malaysia. 
 
2. APPLICATION CONDITIONS
2.1 The applicant must be aged 12 or above. Applicants who are aged 12 or older but not yet 16 must be accompanied by a sponsor. (Sponsor: parent or *guardian. *The guardian could be the grandfather, grandmother, aunt, uncle, elder sister or brother who is over 21.)
2.2 The applicant must go to either the Putrajaya NRD Headquarters or the State NRD Headquarters only.
2.3 Application Form JPN.KP01 (obtainable from the NRD counter) must be completed.
2.4 The application must be submitted within 30 days of the applicant attaining the age of 12, or of the entry permit reference date (oval stamp). Otherwise, a processing fee will be imposed. 
 
3. APPLICATION PROCEDURE
3.1 The applicant is required to go to the counter at the NRD Headquarters or the State NRD Headquarters with the necessary documents (originals and copies).

APPLICATION FOR AMENDMENT OF IDENTITY CARD PARTICULARS
 
1. REASON FOR APPLICATION
1.1 Regulation 14 of the National Registration Regulations 1990 (Amendment 2007) states that the bearer of the identity card who discovers that the identity card contains particulars which to his knowledge are incorrect shall apply to amend the incorrect particulars on the identity card.
 
2. APPLICATION CONDITIONS
2.1 The applicant is required to go to any NRD counter.
2.2 The applicant has an existing identity card. Applicants without an existing identity card may not apply.
2.3 Application Form JPN.KP16 (obtainable from the NRD counter) that is completed.
 
3. APPLICATION PROCEDURE
3.1 The applicant is required to go to any NRD counter with the necessary documents (originals and copies).
 
4. DOCUMENTS REQUIRED (ORIGINALS AND COPIES)

4.1.1 Statutory declaration (if the applicant is under 18, the statutory declaration must be made by the parent or *guardian. *Guardian is the grandfather, grandmother, aunt, uncle, elder sister or brother who is over 21.)

4.1.3 Birth Certificate or Adoption Certificate or Citizenship Certificate or confirmation of citizenship status or entry permit (as applicable)
4.1.4 Passport (if any)
              4.1.5 Other supporting documents relevant to the application, such as:                                                                                               
(i) Card certifying embracing of Islam from the Islamic Religious Department or State Islamic Religious Council for Muslims regardless of name change or name retention; or
(ii) Shariah court order or certification from the Islamic Religious Council or Department for the application to convert from Islam to another religion, and to change from a Muslim name to a non-Muslim one; or
(iii) Baptism certificate or certification that the applicant is a follower of Buddhism, Hinduism or Sikhism for religion change application affecting name. If the original name is retained, then only Appendix “A” needs to be filled at the counter and the above supporting documents will not be required; or
(iv) Marriage Certificate KC07 issued by the NRD for the application to enter the husband’s name for those married to foreign citizens only; or
(v) Other supporting documents required by the department to support the amendment.

5.1 Processing fee for amendment of particulars (citizens and non-citizens): RM10.00
 
5.2 If the application is successful, payment for replacement of the identity card is as follows:
5.2.1 Citizens: RM10.00 
5.2.2 Non-citizens: RM40.00
