CONDITIONS OF APPLICATION
- The applicant must be 12 years old but has not attained the age of 16 years.
- The applicant must be present together with the Sponsor (Mother or Father of Guardian).
- Application Form JPN.KP01 (available at NRD counters) has been completed.
- The applicant must make the application within 30 days from the date the applicant attained the age of 12 years. Otherwise, a compound and a processing fee will be imposed.
APPLICATION PROCEDURES
The applicant is required to go to the NRD Counter with the Sponsor and bringing along the following supporting documents (original) :
- Birth Certificate/ Adoption Certificate/ Citizenship Certificate/ Confirmation of Citizenship Status/ Citizenship Form/ Appendix A (JHEOA) / Entry Permit.
- Identity Card of Sponsor, i.e. Mother or Father or Guardian.
- Statutory Declaration (for Guardian without any family relationship, i.e. other than mother/father/grandfather/grandmother/uncle or aunty or sister or brother who is above 21 years old).
- Passport (for citizen who has just arrived from abroad/ holder of Entry Permit).

CONDITIONS OF APPLICATION
- The applicant must attain the age of 16 years or more.
- The applicant must be present together with the Sponsor (Mother or Father or Guardian).
- The applicant has never applied or possesses an Identity Card and has resided in Malaysia throughout the age of 12 to 16 years.
- The following forms must be completed (available at NRD counters):
(i) Application Form JPN.KP01
(ii) Statutory Declaration (Form JPN DL-1
(iii) Explanatory Form for Late Registration of Identity Card by Mother/Father/Guardian (Form JPN DL-2)
(iv) Extract of School Register (Form JPN D L-3); or
(v) Declaration for Children who have never go to school (Form JPN DL-4)
(vi) Family Information Form (BMK.81 & BMK.81A)
APPLICATION PROCEDURES
The applicant is required to go to the NRD Counter with the Sponsor and bringing along the following supporting documents (original and copy) :
- Birth Certificate/ Adoption Certificate/ Citizenship Certificate/ Confirmation of Citizenship Status/ Citizenship Form/ Entry Permit.
- Identity Card of Sponsor, i.e. Mother or Father or Guardian.

APPLICATION PROCEDURES
The applicant is required to go to the NRD Counter without any document or the applicant can bring the following supporting documents, if any (original and copy):
- Letter of confirmation or Welfare Card from the Social Welfare Department/ Letter of confirmation from the Orang Asli Affairs Department/ Penghulu / Village Chief/ Tribal Chief / District Office/ Chairman of Village Working Committee – for the poor and destitute, and the disabled (OKU).
- Letter of confirmation from the Social Welfare Department/ Fire and Rescue Department/ Penghulu / Village Head/ Tribal Chief/ District Office/ Chairman of the Village Working Committee – for victims of natural disasters.
- Letter of confirmation from Prison/ Rehabilitation Centre – for former prisoners or trainees of the centre.
- Student’s Card or letter of confirmation from School Principal/ Registrar of institution of higher learning – for students from any local or overseas school, college or institution of higher learning.
- Police report which states clearly the particulars of the incidence and the lost Identity Card.

If an application is made for a replacement MyKad within 12 months from the date of delivery of the card, no payment is imposed (FREE).

Information: According to Regulation 13, National Registration Regulations 1990 (Amendment 200), every person whose Identity Card is damaged or destroyed is required to apply for replacement Identity Card immediately.
CONDITIONS OF APPLICATION
- The applicant must come personally.
- Change of Address Form (available at NRD counters) has been completed.
- The address must be the address of permanent residence in Malaysia. Addresses of work place, office or post office box are not allowed.

Information : According to Regulation 15, National Registration Regulations 1990 (Amendment 2001), every person is required to apply for a replacement Identity Card if he/she changes residence where he/she will reside for 90 days or more.
CONDITIONS OF APPLICATION
- The applicant must be 16 years old or more.
- The applicant must come personally.
- The applicant was not in Malaysia throughout the ages of 12 to 16 (Passport as proof of travel).
- Application Form JPN.KP01 (available at NRD counters) has been completed.
- The applicant must make the application within 30 days from the date of arrival in Malaysia. Otherwise, a compound and processing fee shall be imposed.

- The applicant must make the application within 30 days from the reference date in the Entry Permit (oval stamp). Otherwise, a compound and a processing fee shall be imposed.
- Applications are only accepted at NRD Headquarters in Putrajaya and State NRD Headquarters.

- The applicant must come personally.
- The applicant possesses an Identity Card. An applicant who has yet to possess an Identity card is not eligible to apply.
- Application Form JPN.KP16 (available at NRD counters) has been completed.
APPLICATION PROCEDURES
The applicant is required to go the NRD Counter and bring the following documents (original and copy) :
- Statutory Declaration (if the applicant is below the age of 18, the declaration shall be made by the mother/father/guardian).
- Identity Card.
- Birth Certificate/ Adoption Certificate/ Citizenship Certificate/ Confirmation of Citizenship Status/ Citizenship Form/ Entry Permit (where applicable).
- Copy of Personal Identification Document of Mother/ Father/ Guardian for applicants who are below the age of 18 (copy of Identity Card or Temporary Identification Document).
- Passport (if any).
- Other supporting documents pertaining to the application such as:
- Islamic Conversion Card for those who have converted to Islam irrespective of whether to apply for a change in name or to retain the original name.
- Syariah Court Order/Confirmation of the Islamic Religious Council or Department for application to change religion from Islam to non-Islam and to change an Islamic name to a non-Islamic name.
- Baptism Certificate / Letter of confirmation of conversion to Buddhism / Hinduism / Sikhism for application to change religion which involves change in name. To retain the original name, the applicant is only required to complete Appendix “A” at the counter and the application will be accepted without having to submit the above supporting documents.
- Marriage Register KC07 issued by NRD for application to enter name of husbands for those who are married to foreign citizens only.
- Other documents required by the Department to support the alteration application.
CASH/ ELECTRONIC PAYMENT
- Citizen/ Non-Citizen= RM 10.00 (Alteration of Particular processing fee)
If the Alteration of Particulars application is approved, the following Replacement Card application fee shall be imposed:
- Citizen = RM 10.00
- Non-Citizen = RM 40.00
Information: According to Regulations 14 (1) & (2), National Registration Regulations 1990 (Amendment 2001), every person who apply to change his/her name or to correct the particulars in the Identity Card other than his/her address is required to apply for a replacement Identity Card immediately.
