EXTRACT OF BIRTH CERTIFICATE
 
Application to obtain copy of birth register by qualified applicant on payment of the prescribed fees.

APPLICATION PROCEDURES
 
Applicants need to complete form JPN.LM12 and attach the supporting documents. Registration can be made any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS
 
1.       Personal identification document or valid travel document (if non-citizen)
2.      Departmental Official Letter to apply for a search of the register (for application from Government Department)
3.       Any document or information that can be used as a basis to make a search of the birth register such as
         Extract of School Register and Birth Certificates of other siblings (for birth search application)
