Delayed Registration of Death
DELAYED REGISTRATION OF DEATH
Delayed registration of death application is for death occurring in Malaysia and registered within 8 to 14 days from the date of death. However, body that is not found CANNOT BE REGISTERED.

APPLICATION PROCEDURES
Applicants need to complete Form JPN.LM02 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.
SUPPORTING DOCUMENTS
1. Certificate of Death JPN.LM09/JPN.LM10 (if death occurs in a hospital) or Burial Permit
2. Personal identification document of informant (non-citizen)
3. Personal identification of the deceased (if any)
4. Form JPN.LM25 / JPN.LM26 if identity card/Certificate or Extract of Birth Register of the deceased is not submitted.
