EXTRACT OF DEATH CERTIFICATE IN HEDJAZ

Application to obtain copy of death register by qualified applicant on payment of the prescribed fees.

APPLICATION PROCEDURES
 
Applicants need to complete Form JPN.LM14 and attach the supporting documents. Application can be made at any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

1.      Copy of Death Certificate/ Report (if any)
2.      Original Personal Identification Document of the Deceased (if any)
3.      Form JPN.LM26
4.      Original or certified copy of Personal Identification Document or valid travel document (non-citizen)
