Registration of Death by Natural Causes

REGISTRATION OF DEATH BY NATURAL CAUSES

Application for registration of death by natural causes occurring in Malaysia and registered within 7 days from the date of death.

APPLICATION PROCEDURES

Applicants need to complete Form JPN.LM02 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

1. Original Identity Card or personal identification of the deceased
2. Identity Card or personal identification of the informant
3. Proof of death documents such as burial permit
4. Certification by Medical Practitioner as to the cause of death [Form JPN.LM09 or JPN.LM10 (Post Mortem)]

FEES

No application fee.
