NORMAL REGISTRATION OF DEATH
(SABAH)
 
REASON FOR APPLICATION 
Normal registration of death refers to registration of a death in Sabah that is made within 24 hours of death.
 
APPLICATION CONDITION
Application for registration of death may be made by the following persons:
1. Next of kin of the deceased;
2. Person with knowledge of the death
 
REQUIRED DOCUMENTS 
1. Death registration form B and form N2 (extract) that are completed;
2. Police report for death occurring at home (original and copy);
3. Certification of death if the death occurred in a hospital (original and copy);
4. Identity card or entry permit or passport of the next of kin of the deceased or of the person with knowledge of the death (original and copy);
5. Original identity card or identification document of the deceased. If these are unavailable/lost then a police report must be included.

DELAYED REGISTRATION OF DEATH 
(SABAH)
 
REASON FOR APPLICATION 
Application for delayed registration of death is required when registration is made on the second to the tenth day after the death. 
 
APPLICATION CONDITION
Death registration application may be made by the following persons:
1. Next of kin of the deceased; 
2. Person with knowledge of the death
 
REQUIRED DOCUMENTS 
1. Forms B and N2 (extract) that are completed;
2. Police report for death occurring at home (original and copy);
3. Certification of death if the death occurred in a hospital (original and copy);
4. Identity card or entry permit or passport of the informant (original and copy);
5. Identity card or identification document of the deceased. If these are unavailable/lost then a police report must be included.

Processing fee: RM5.00

LATE REGISTRATION OF DEATH
(SABAH)
 
REASON FOR APPLICATION 
Application for late registration of death is required when registration is made after the tenth day of death in Sabah. 
 
APPLICATION CONDITION
An application for late registration of death may be submitted by the following persons:
1. Mother of the deceased;
2. Father of the deceased;
3. Child of the deceased;
4. Sibling of the deceased;
5. Spouse of the deceased;
6. Investigating police officer if there is no next of kin of the deceased
 
REQUIRED DOCUMENTS 
1. Forms B and N2 (extract);
2. Identification document of the deceased;
3. Identity card or identification document of the informant (original and copy);
4. Police report for death occurring at home or certification from a medical practitioner for death occurring in a hospital/clinic;
5. Declaration of the native chief or headman and witness’s testimony

APPLICATION TO CORRECT CLERICAL ERRORS [SECTION 31 (2)] IN THE DEATH CERTIFICATE
(SABAH)
 
Application to correct spelling errors which even if not corrected will not change the facts in the Death Certificate. 
 
APPLICATION CONDITION
Application for correction of information in Section 31 (2) may be submitted by the following persons:
1. Parent of the deceased;
2. Spouse of the deceased;
3. Sibling of the deceased;
4. Child or legally adopted child of the deceased
 
REQUIRED DOCUMENTS 
1. Forms PSB 4 and PSB 4A that are completed;
2. Death Certificate of the deceased (original and copy);
3. Identity card or identification document of the applicant (original and copy);
4. Marriage certificate or document if application is made by the spouse;
5. Other supporting documents
 
PAYMENT
1. Registration fee: RM5.00;
2. Fee for extract of Death Certificate: RM10.00

EXTRACT OF DEATH CERTIFICATE 
(SABAH)
 
REASON FOR APPLICATION 
Application by an eligible applicant to obtain a copy of the death register by paying a prescribed fee. Application for extract of the Death Certificate can be made only at the State NRD Office and Putrajaya NRD.
   
APPLICATION CONDITION
Application for extract of the Death Certificate may be submitted by the following persons or entities:
A. Parent of the deceased;
B. Spouse of the deceased;
C. Child or legally adopted child of the owner of the Death Certificate;
D. Sibling of the owner of the death certificate;
E. Law firm;
F. Statutory body;
G. Government agency
 
REQUIRED DOCUMENTS 
CATEGORY 1: PERSON IN CATEGORIES A TO D
1. Form N2 that is completed;
2. Identity card or identification document of the applicant (original and copy);
3. Copy of Death Certificate of the deceased (if any)
 
CATEGORY 2: PERSON IN CATEGORY E
1. Form N2 that is completed;
2. Official letter from a law firm requesting search or extract of the register;
3. Letter of appointment from the client (applicant);
4. Identity card or identification document of the client (applicant) (copy);
5. Power of attorney to oversee the application (if applicable)
 
CATEGORY 3: PERSON IN CATEGORIES F AND G 
1. Form N2 that is completed;
2. Official letter from a statutory body or government agency to apply for a search or extract of the register;
3. Power of attorney to oversee the application (if applicable)
 
PAYMENT
Application fee is RM5.00. Government agencies are exempted from payment.

REPORT OF DEATH ABROAD 
(SABAH)
 
REASON FOR APPLICATION 
Death of Malaysian citizens in a foreign country must be reported to the National Registration Department (NRD). The department will input information on the death in the NRD system and certification of death will be issued to the applicant.
 
The applicant is required to complete Form B (update of death abroad) and submit supporting documents. Deaths abroad can be reported at any NRD office in Sabah.
 
APPLICATION CONDITION
Application to report a death in a foreign country may be submitted by the following persons:
1. Spouse of the deceased;
2. Child of the deceased aged 18 years and above;
3. Parent of the deceased;
4. Sibling of the deceased aged 18 years and above; 
5. Person with knowledge of the death and able to provide information or evidence of the death
 
REQUIRED DOCUMENTS 
1. Form B (update of death abroad);
2. Original identity card or identification document of the deceased;
3. Identity card or identification document of the applicant (original and copy);
4. Documented proof of death:- 
4.1 Death Certificate issued by the country where death occurred (original and copy);
4.2 Translation into Malay or English if the Death Certificate is issued in a language other than Malay or English (original and copy);
4.3 Certification of death from the clinic or hospital abroad where death occurred (original and copy);
4.4 Certification of death from the Malaysian Embassy in the foreign country where death occurred (original and copy)

REPORT OF PERSON PRESUMED DEAD 
(SABAH)
 
REASON FOR APPLICATION 
Section 8 of the Registration of Births and Deaths Ordinance (Sabah Cap. 123) clearly states that the registration of death of a person in Malaysia is allowed only on condition that the body is found. For persons that the High Court has classified as presumed to be dead, the court will issue an order declaring that the person is legally presumed dead. 
 
The NRD will not register or issue a death certificate for a person presumed dead. However, it will update the NRD system with information of the person that the High Court has declared to be presumed dead. For matters relating to a person who has been presumed dead, the next of kin must use the court order issued by the High Court.
 
Application to update the information of persons who are presumed dead by court order will be handled only by the NRD headquarters in Putrajaya. The application may also be submitted at the NRD of a state capital to be forwarded to the NRD headquarters in Putrajaya.
 
APPLICATION CONDITION
Application for report of person presumed dead may be submitted by the following persons or entities:
1. Parent of the person whose name appears in the court order;
2. Spouse of the person whose name appears in the court order;
3. Child or legally adopted child of the person whose name appears in the court order;
4. Sibling of the person whose name appears in the court order;
5. Law firm
 
REQUIRED DOCUMENTS 
1. Form B that is completed;
2. Court order from the High Court or Shariah High Court or Court of Appeals or Federal Court that the person is presumed dead (a court order for presumption of death from courts other than those stated will not be accepted);
3. Application letter from the law firm (if the applicant is a law firm);
4. Identity card or identification document of the applicant (original and copy);
5. Identity card of the client (if the applicant is a law firm) (copy);
6. Identity card of the deceased
