Extract of Birth Certificate

EXTRACT OF BIRTH CERTIFICATE
 
Application to obtain copy of the birth register by qualified applicant upon payment of the prescribed fees. The informant must be the mother/father of the child. Applications may be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

APPLICATION PROCEDURES
 
Applicants need to complete Form N1 (Extract) and attach the supporting documents.
 
SUPPORTING DOCUMENTS
 
1.       Personal identification document or valid travel document (if non-citizen)
2.      Departmental Official Letter to apply for a search of the register (for application from Government Department)
3.       Any document or information that can be used as a basis to make a search of the birth register such as Extract of the School Register and Birth Certificates of other siblings (for birth search application)
