Registration of Stillbirth

REGISTRATION OF STILLBIRTH

 Applicants need to register a stillbirth within 14 days from the date of birth and the informant must be the mother/father/relative/person who knew of the birth. Applications may be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

APPLICATION PROCEDURES
 
Applicants need to complete Forms A and N1 (Extract) and attach the supporting documents.
 
SUPPORTING DOCUMENTS
 
1.     Personal identification document or valid travel document (if mother/father/informant is not a citizen)
2.      Expectant Mother Examination Document
3.      Confirmation of Stillbirth
4.      Marriage Document of Parents (for first child only) – if married

 
FEES
 
No fee is imposed.
