Late Registration
LATE REGISTRATION
Late birth registration application means the registration of every birth occurring in Malaysia after 42 days from the date of birth of the child.

APPLICATION PROCEDURES
Late birth registration application can be made at any NRD Office throughout Peninsular Malaysia by submitting the completed Form I and attaching the supporting documents. If the parents have passed away, any person aged at least 10 years or older than the applicant who knew of the birth. However, the application will be forwarded to Sarawak for the approval of the Regional Registrar. Each application made will be accepted after the result of the search is obtained by using Form X (Extract).

1. Personal Identification document or valid travel document (if mother/father/informant is not a citizen)
2. Expectant mother examination card/book (if any)
3. Birth or stillbirth confirmation form (if any)
4. Marriage Document (if any)
5. Late Registration Application Statutory Declaration SA81
6. Family Information Form – BMK81
7. List of Photographs of Father, Mother and Siblings – BMK81A
8. Extract of Birth Certificate Form XII
