Late Registration of Death

LATE REGISTRATION OF DEATH

Applicants who register a death after 7 days from the date of the death and the informant must be the immediate family members of the deceased (mother/father/relative/wife/husband). Applications can be made in Peninsular Malaysia and will be forwarded to Sarawak for the approval of the Regional Registrar.

APPLICATION PROCEDURES

Applicants need to complete Forms III and XI and attach the supporting documents.

SUPPORTING DOCUMENTS
  
1.      Confirmation of Community Leader/Tribal Chief for Sarawak
2.      Form VIII (Medical Certificate of the Cause of Death)
4.      Form IX (After Post-Mortem)
5.     Personal Identification Document of informant or valid travel document (if mother/father/informant is not a citizen))
        Personal Identification Document of the deceased
4.      Police Report if identity card/certificate or extract of Birth Register of the deceased is not submitted
