NORMAL REGISTRATION OF BIRTH (SABAH)
 
REASON FOR APPLICATION 
Normal registration of birth refers to registration that is made within 14 days of the birth of a child in Malaysia.

APPLICATION CONDITION

Application for registration of a child may be submitted by the following persons:
1. Parent;
2. Legal guardian;
3. Person having knowledge of the birth
 
APPLICATION PROCEDURE
The applicant is required to go to any National Registration Department counter with the required documents. 
 
REQUIRED DOCUMENTS (ORIGINALS AND COPIES)
CHILD’S DOCUMENTS (ORIGINALS AND COPIES)
1. Forms A and N1 (extract) that are completed;
2. Confirmation of birth form from the hospital or certification of home birth from the midwife/doctor (original); 
3. Prenatal card (maternity examination book) (original and copy); and
4. Child’s health and immunisation card (original and copy)
 
PARENTS’ DOCUMENTS (ORIGINALS AND COPIES)
1. Identity cards or entry permits or passports of the parents and the person reporting the birth (originals and copies);
2. Marriage or divorce document or death certificates of the parents (originals and copies)
 
INFORMANT’S DOCUMENT (ORIGINAL AND COPY)
1. Identity card or entry permit or passport (original and copy)

DELAYED REGISTRATION OF BIRTH (SABAH)
 
REASON FOR APPLICATION 
Delayed registration of birth refers to registration that is made from the 15th to the 42nd day after the birth of a child.
  
APPLICATION CONDITION
Application for registration of a child may be submitted by the following persons:
1. Parent;
2. Legal guardian;
3. Person having knowledge of the birth
 
APPLICATION PROCEDURE
The applicant is required to go to any National Registration Department counter with the required documents. 
 
REQUIRED DOCUMENTS (ORIGINALS AND COPIES)
CHILD’S DOCUMENTS (ORIGINALS AND COPIES)
1. Forms A and N1 (extract) that are completed;
2. Confirmation of birth form from the hospital or certification of home birth from the midwife/doctor (original);

LATE REGISTRATION OF BIRTH (SABAH)

REASON FOR APPLICATION 
Late registration of birth refers to registration made more than 42 days after the date of birth of a child in Malaysia.  
 
APPLICATION CONDITIONS
ELIGIBILITY OF THE APPLICANT, SPONSOR OR INFORMANT
1. Parent;
2. If the father is not available, then the mother or a sponsor with knowledge of the birth who is 10 years older than the child;
3. If the father is not available and the mother has died, the application may be submitted by two (2) sponsors with knowledge of the child, who are both aged 18 and above and are at least 10 years older than the child;
4. If the newborn is found exposed and its birth was not registered, then the application may be submitted by two (2) sponsors who are social welfare officers or persons given custody of the child in accordance with the Child Protection Act 1991
 
APPLICATION PROCEDURE
Application for late registration of birth can be made throughout Sabah by bringing along the necessary documents to any NRD office near the applicant's place of residence.
 
REQUIRED DOCUMENTS (HOME BIRTH)
CHILD’S DOCUMENTS (ORIGINALS AND COPIES)
1.   Birth registration search form N1 (for births from 2002 onwards);
2.   Birth registration form (Form A) that is completed;
3. Prenatal card or maternity examination book, child’s health and immunisation card, certification of home birth by midwife/doctor (originals and copies). If unavailable, a statutory declaration is required;
4. Recent full-length 4R photograph (one (1));
5. Recent passport-sized photograph (one (1));
6. Certification of birth from a headman or village chief or Member of Parliament or member of a State Legislative Assembly for the constituency where the child was born; 
7. Extract of school register or school certificate with endorsement from the head teacher or school principal if the child is schooling and aged seven (7) and above, or alternatively, certification from a headman or village chief or Member of Parliament or member of a State Legislative Assembly for the constituency where the child resides, that the child has never been schooled and is aged seven (7) and above (if any);
8. Statutory declarations of two (2) witnesses with knowledge of the birth

1.   Birth certificate or identity card, if any (original and copy); 
2.   Recent passport-sized photograph of the sibling from the same parents (one (1));
3.   Family information forms BMK81 and BMK81A
 
SPONSOR’S DOCUMENTS (ORIGINALS AND COPIES)
1.   Statutory declaration JPN.LM27 of two (2) sponsors with knowledge of the birth;
2.  Identity card or entry permit or passport or death certificate of the informant or witness or sponsor (original and copy)
 
REQUIRED DOCUMENTS (BIRTH IN GOVERNMENT/PRIVATE HOSPITAL OR CLINIC)
CHILD’S DOCUMENTS (ORIGINALS AND COPIES)
1. Birth registration search or extract form  – form N1;
2. Birth registration form A;
3. Confirmation of birth form;
4. Prenatal card or maternity examination book, child’s health and immunisation card, certification of home birth by midwife/doctor (originals and copies);
5. Recent full-length 4R photograph (one (1));
6. Recent passport-sized photograph (one (1));
7. 1.   Statutory declaration JPN.LM27 of two (2) sponsors with knowledge of the birth;
2.  Identity card or entry permit or passport or death certificate of the informant or witness or sponsor (original and copy)

REGISTRATION OF STILLBIRTH
 
The applicant must register the stillbirth of a child within 14 days of the date of birth. The informant must be the mother/father/relative/person who knows about the birth. The application can be made in Peninsular Malaysia, which will be forwarded to Sabah for the approval of the Regional Registrar.

APPLICATION PROCEDURE
 
The applicant must complete Form A and Form N1 (Extract) and attach supporting documents.
 
SUPPORTING DOCUMENTS
 
1.    Personal identification document or a valid travel document
 
2.    Pre-natal examination documents
 
3.    Confirmation of stillbirth
 
4.    Marriage document of the parents (for firstborn only) - if married.

APPLICATION FOR SEARCH IN REGISTER

An application for search is to obtain the result from the register of births on payment of the prescribed fee. The informant must be the mother/father/guardian of the child. The application can be made in Peninsular Malaysia, which will be forwarded to Sabah for the approval of the Regional Registrar.

APPLICATION PROCEDURE

The applicant must complete Form N1 (Extract) and attach supporting documents.

 

SUPPORTING DOCUMENTS

1.    Personal identification document or a valid travel document (for a non-citizen)

2.    Official letter from the department to apply for search in the register (for applications made by a government department)

3.    Any document or information that can be used as the basis for a search in the register of births such as extract of school register and birth certificates of siblings (for the search on birth)

EXTRACT OF BIRTH CERTIFICATE
(SABAH)
 
REASON FOR APPLICATION 
Application by an eligible applicant to obtain a copy of the Birth Certificate by paying a prescribed fee. Extract of the Birth Certificate can be made only at the State NRD Office and Putrajaya NRD.
 
ELIGIBLE APPLICANTS
A. Birth Certificate owner;
B. Parent of Birth Certificate owner;
C. Spouse of Birth Certificate owner;
D. Child or legally adopted child of Birth Certificate owner;
E. Sibling of Birth Certificate owner;
F. Law firm;
G. Statutory body;
H. Government agency
 
REQUIRED DOCUMENTS 
CATEGORY 1: PERSON IN CATEGORIES A TO E 
1. Form N1 that is completed;
2. Identity card or identification document (original and copy);
3. Copy of Birth Certificate of owner (if available)
 
CATEGORY 2: PERSON IN CATEGORY F
1. Form N1 that is completed;
2. Official letter from a law firm requesting search or extract of the register;
3. Letter of appointment from client (applicant);
4. Identity card or identification document of client (applicant) (original and copy);
5. Power of attorney to oversee the application (if applicable)
 
CATEGORY 3: PERSON IN CATEGORIES G AND H 
1. Form N1 that is completed;
2. Official letter from a statutory body or government agency to apply for a search or extract of the register;
3. Power of attorney to oversee the application (if applicable)
 
PAYMENT
Application fee is RM5.00. Government agencies are exempted from payment.

APPLICATION TO CORRECT CLERICAL ERRORS [SECTION 30] IN THE BIRTH CERTIFICATE
(SABAH)
 
REASON FOR APPLICATION 
Application to correct minor errors made by the Registrar involving spelling errors only which even if not corrected will not change the facts in the Birth Certificate. 
 
APPLICATION CONDITION
Application to correct the clerical errors may be submitted by the following persons:
1. Parent;
2. Legal guardian
 
APPLICATION PROCEDURE
The applicant is required to go to any National Registration Department counter in Sabah with the required documents. 
 
REQUIRED DOCUMENTS 
CHILD’S DOCUMENTS (ORIGINALS AND COPIES)
1. Forms PSB 4 and PSB 4A that are completed; 
2. Birth Certificate (original and copy);
3. Identity card or identification document (original and copy);
4. Passport or insurance documentation or bank book if the child has a passport or insurance or bank account (original and copy);
5. Any other supporting documents

2.   Authorisation letter from the parents for the guardian to submit the application

APPLICATION TO CORRECT FACTUAL ERRORS [SECTION 30] IN THE BIRTH CERTIFICATE
(SABAH)
 
REASON FOR APPLICATION 
Application to correct any factual errors other than the name of a child provided during the registration of birth due to incorrect or unavailable information. 
 
ELIGIBLE APPLICANTS
Application to correct factual errors may be submitted by the following persons:
1. Birth Certificate owner;
2. Parent;
3. Legal guardian;
4. Two (2) persons who are acquainted with the child
 
APPLICATION PROCEDURE
The applicant is required to go to any National Registration Department counter in Sabah with the necessary documents. 
 
REQUIRED DOCUMENTS 
CHILD’S DOCUMENTS (ORIGINALS AND COPIES)
1. Forms PSB 4 and PSB 4A that are completed; 
2. Statutory declaration;
3. Family information form BMK81;
4. MyKid form;
5. Birth Certificate (original and copy);
6. Identity card or identification document (original and copy);
7. Passport or insurance documentation or bank book if the child has a passport or insurance or bank account (original and copy);
8. 3R (postcard) and passport-sized photographs
 
PARENTS’ DOCUMENTS (ORIGINALS AND COPIES)
1.   Identity card or identification document (original and copy);
2.   Authorisation letter from the parents for the guardian to submit the application;
3.   Marriage or divorce or death certificates of parents (originals and copies);
4. Letter of guardianship from the Department of Social Welfare (if application is submitted by the child’s guardian) (original and copy);
5. Other supporting documents such as the certificate or card confirming embracing of Islam of a parent, birth or death certificate of a parent and the letter of approval to change name from the Identity Card Division of NRD
