REGISTRATION OF DEATH BY NATURAL CAUSES
Application for registration of death by natural causes occurring in Malaysia and registered within 7 days from the date of death.
APPLICATION PROCEDURES
Applicants need to complete Form JPN.LM02 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.
SUPPORTING DOCUMENTS
- Original Identity Card or personal identification of the deceased
- Identity Card or personal identification of the informant
- Proof of death documents such as burial permit
- Certification by Medical Practitioner as to the cause of death [Form JPN.LM09 or JPN.LM10 (Post Mortem)]

No application fee.

LATE REGISTRATION OF DEATH
Late registration of death application is for death occurring in Malaysia and registered after 14 days from the date of death. However, body that is not found CANNOT BE REGISTERED.

APPLICATION PROCEDURES
Applicants need to complete Forms JPN.LM02 and JPN.LM28 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia. Application is accepted after the result of the search is obtained (Form JPN.LM14) except if the death occurs after 2003. However, body that is not found CANNOT BE REGISTERED.

2. Personal identification document of the informant (if non-citizen)
3. Certificate of Death JPN.LM09/JPN.LM10 (If death occurs in a hospital)

· Compound not exceeding RM 50.00 if application is approved.
DELAYED REGISTRATION OF DEATH
Delayed registration of death application is for death occurring in Malaysia and registered within 8 to 14 days from the date of death. However, body that is not found CANNOT BE REGISTERED.

Applicants need to complete Form JPN.LM02 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.

1. Certificate of Death JPN.LM09/JPN.LM10 (if death occurs in a hospital) or Burial Permit
2. Personal identification document of informant (non-citizen)
3. Personal identification of the deceased (if any)
4. Form JPN.LM25 / JPN.LM26 if identity card/Certificate or Extract of Birth Register of the deceased is not submitted.

Processing fee: RM 5.00
REGISTRATION OF DEATH WHILE ON PILGRIMAGE
Every death occurring while on pilgrimage should be reported to the registrar in Hedjaz within 40 days from the date of death or reported to the NRD Headquarters counter within 6 months from the date of death.

APPLICATION PROCEDURES
Hedjaz death registration application can be made at any NRD Office throughout Peninsular Malaysia by submitting Form JPN.LMH2 and attaching the supporting documents. However, approval is only made at NRD Headquarters.

2. Personal Identification Document of the deceased
3. Personal Identification Document of the Informant (if informant is not the parents)
FEES
No fee is imposed.
CORRECTING ERROR OF INFORMATION IN CERTIFICATE OF DEATH
i. APPLICATION TO CORRECT CLERICAL ERROR
Application to correct minor error which only involves spelling error which if corrected does not alter the fact in the Certificate of Death.

APPLICATION PROCEDURES
Applicants need to complete Forms JPN.LM21 and JPN.LM22 and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.
SUPPORTING DOCUMENTS
1. Personal identification documents of parents (if non-citizen)
2. Personal identification document of guardian (if non-citizen) proven by Letter of Permission of Guardianship from JKM
3. Letter of authorization from natural parents who give permission to guardian to make alteration application
4. Personal identification document of applicant (if non-citizen) who exceeds 21 years old
5. Personal identification document of the next-of-kin of the deceased/applicant (if non-citizen)
6. Other documents proving errors
FEES
Application fees:
· RM 2.00 on submission of application.
· RM 5.00 for print-out of extract of Death Certificate.
ii. APPLICATION TO CORRECT ERROR OF FACT
Application to correct error of fact where the information other than the name of the deceased provided by the informant during registration of the death is incorrect or not available. The applicant needs to submit supporting documents to prove the error of fact.

APPLICATION PROCEDURES
Applicants need to complete Form JPN.LM21, JPN.LM22 and Statutory Declaration and Form JPN.LM21 and attach supporting documents.

EXTRACT OF DEATH CERTIFICATE
Application to obtain copy of death register by qualified applicant on payment of the prescribed fees.

APPLICATION PROCEDURES
Applicants need to complete Form JPN.LM13 and attach the supporting documents. Application can be made at any NRD Office throughout Peninsular Malaysia.
SUPPORTING DOCUMENTS
1. Personal identification document or valid travel document (if non-citizen)
2. Departmental Official Letter to apply for a search of the register (for application from Government Department)
3. Any document or information that can be used as a basis to make a search of the death register such as Burial Permit/Police Report.

EXTRACT OF DEATH CERTIFICATE IN HEDJAZ
Application to obtain copy of death register by qualified applicant on payment of the prescribed fees.

EXTRACT OF DEATH CERTIFICATE OUTSIDE THE COUNTRY
Application to obtain copy of death register by qualified applicant on payment of the prescribed fees.

Applicants need to complete Form JPN.LM02 and attach the supporting documents. Application can be made at any NRD Office throughout Peninsular Malaysia.
Note: Letter of Confirmation of Death will be issued to applicants.

1. Original Certificate of Death issued by the country concerned that has been translated into Malay/ English
2. Personal Identification Document of the deceased
3. Certificate of Birth of the deceased (if any)
4. Personal Identification Document of the applicant
5. Certificate of death by High Commission/ Embassy/ Consul Office of Malaysia in the country concerned.
