NORMAL REGISTRATION OF DEATH

Applicants who register a death within 24 hours from the date of the death and the informant must be the mother/father/relative/person who knew of the death. Application can be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

APPLICATION PROCEDURES

Applicants need to complete Forms B and N2 (Extract) and attach the supporting documents.

SUPPORTING DOCUMENTS

1.      Police Report for death which occur in a house
2.      Certificate of death for death which occur in a hospital
3.      Personal Identification Document of informant or valid travel document (if mother/father/informant is not a citizen))
4.      Police Report if identity card/certificate or extract of the Birth Register of the deceased is not submitted

FEES

No fee is imposed.

LATE REGISTRATION OF DEATH

Applicants who register a death after the end of the 24 hours to 10 days period from the date of the death and the informant must be the mother/father/relative/person who knew of the death. Applications can be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

APPLICATION PROCEDURES

Applicants need to complete Forms B and N2 (Extract) and attach the supporting documents.

SUPPORTING DOCUMENTS

1.      Personal Identification Document of the Deceased
2.      Personal Identification Document of the Informant (if non-citizen)
3.      Burial Permit
4.      Police Report for death which occur in a house
5.      Statement of Native Chief/Village Head and Witness Declaration

DELAYED REGISTRATION OF DEATH

Applicants who register a death within 24 hours to 10 days from the date of the death and the informant must be the mother/father/relative/person who knew of the death. Applications can be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

APPLICATION TO CORRECT CLERICAL ERROR

Application to correct minor error which only involves spelling error which if corrected does not alter the fact in the Certificate of Death.

APPLICATION PROCEDURES

 Applicants need to complete Forms PSB 4 and PSB 4A and attach the supporting documents. Applicants should be 2 next-of-kins of the deceased. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.

SUPPORTING DOCUMENTS

1.     Personal identification documents of parents (if non-citizen)
2.      Personal identification of guardian (if non-citizen) proven by Letter of Permission of Guardianship from JKM
3.      Letter of authorization from natural parents who give permission to guardian to make alteration application
4.      Personal identification document of applicant (if non-citizen) who exceeds 21 years old
5.     Personal identification document of the next-of-kin of the deceased/applicant (if non-citizen)
6.      Other documents proving errors

EXTRACT OF DEATH CERTIFICATE

Application to obtain copy of the death register by qualified applicant on payment of the prescribed fees.

Applicants need to complete Forms P and N2 and attach the supporting documents. Applications can be made in Peninsular Malaysia and will be forwarded to Sabah for the approval of the Regional Registrar.

SUPPORTING DOCUMENTS

1.   1.      Personal identification document or valid travel document (if non-citizen)
2.      Departmental Official Letter to apply for a search of the register (for application from Government Department)
3.       Any document or information that can be used as a basis to make a search of the death register such as Burial Permit/Police Report.
