Death : Sarawak
NORMAL REGISTRATION OF DEATH
Applicants need to register a death within 24 hours from the date of the death and the informant must be the closest family members of the deceased (mother/father/relative/wife/husband). Applications can be made in Peninsular Malaysia and will be forwarded to Sarawak for the approval of the Regional Registrar.

APPLICATION PROCEDURES
Applicants need to complete Forms III and XI and attach the supporting documents.
SUPPORTING DOCUMENTS
1. Confirmation of Community/Tribal Leader for Sarawak
2. Police Report for death which occur in a house
3. Form VIII (Medical Certificate of the Cause of Death)
4. Form IX (After Post-Mortem)
5. Personal Identification Document of informant or valid travel document (if mother/father/informant is not a citizen))

4. Police Report if identity card/certificate or extract of the Birth Register of the deceased is not submitted
FEES
No fee is imposed.
LATE REGISTRATION OF DEATH
Applicants who register a death after 7 days from the date of the death and the informant must be the immediate family members of the deceased (mother/father/relative/wife/husband). Applications can be made in Peninsular Malaysia and will be forwarded to Sarawak for the approval of the Regional Registrar.

AMENDMENT OF PARTICULAR OF DEATH
i. APPLICATION TO CORRECT CLERICAL ERROR
The application must be to correct error in the death register which is categorised as clerical error. The applicant must be the mother/father/children/siblings/wife/husband. Applications can be made in Peninsular Malaysia and will be forwarded to Sarawak for the approval of the Regional Registrar

APPLICATION PROCEDURES
Applicants need to complete Forms XI and JPN.LM21 and attach the supporting documents.
SUPPORTING DOCUMENTS
1. Personal identification documents of parents (if non-citizen)
2. Personal identification document of guardian (if non-citizen) proven by Letter of Permission of Guardianship from JKM
3. Letter of authorization from natural parents who give permission to guardian to make alteration application
4. Personal identification document of applicant (if non-citizen) who exceeds 21 years old
5. Personal identification document of the next-of-kin of the deceased/applicant (if non-citizen)
6. Other documents proving errors

ii. APPLICATION TO CORRECT ERROR OF FACT
Application to correct error of fact where the information other than the name of the deceased provided by the informant during registration of the death is incorrect or not available. The applicant needs to submit supporting documents to prove the error of fact.

Applicants need to complete Form X, Statutory Declaration SA/81 (if required) and Form JPN.LM21 and attach the supporting documents.

EXTRACT OF DEATH CERTIFICATE
Extract of death certificate application for registry owner who has not been issued a death certificate/ application for replacement of death certificate that is missing/destroyed. The applicant must be the next-of-kin of the deceased/wife/husband/children/legal firm. Applications can be made in Peninsular Malaysia and will be forwarded to Sarawak for the approval of the Regional Registrar

APPLICATION PROCEDURES
Applicants need to complete Form XI and attach the supporting documents. Registration of death can be made at any NRD Office throughout Peninsular Malaysia.
SUPPORTING DOCUMENTS
1. Personal identification document or valid travel document (if non-citizen)
2. Departmental Official Letter to apply for a search of the register (for application from Government Department)
3. Any document or information that can be used as a basis to make a search of the death register such as Burial Permit/Police Report.
