Application For Amendment of Particulars On Citizenship Documents

Any person who wishes to ammend of the Citizenship document particulars due of error mistake may do so by making a written application using the prescribed form.

Form P must be completed and submitted together with A4 copies of the necessary documents.

Applications may be submitted at the counter of the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters/branches or by post.

  1. Identity card or extract particular applicant’s information  on identity card
  2. Citizenship Certificate/Confirmation Status Citizenship Certificate/ Form W/Form K (Originals)
  3. Supported documents as a proof for amendment
  4. 3pc Passport-sized photograph of the applicant with a blue background
  5. Birth Certificate or latest extract of Birth Certificate (if any)
  6. Marriage document (if any)
  7. Only copies of documents need to be sent for applications made through the post
  8. The applicant shall be the:

8.1. Document owner or;
8.2. Parent of the document owner or;
8.3. Spouse of the document owner or;
8.4. Child of the document owner or ;
8.5. Next of kin (grandfather/grandmother only)

 

  1. RM10.00 for application fee
  2. RM10.00 to issue the document

Note: Payment is by cash or postal order of RM10.00 value.

Application For Renunciation Of Citizenship Under Article 23 Of The Federal Constitution

Any citizen aged twenty-one years and above and of sound mind and who already is, or is about to become a citizen of another country may renounce his/her Malaysian citizenship. Age will not be taken into account if the woman is married.

1. The applicant must be aged 21 and above.

2. The applicant is of sound mind.

3. The applicant has or will soon obtain foreign citizenship.

4. Two copies of Form K must be completed and submitted together with copies of the necessary documents.

The applicant is required to go to any counter of the Citizenship Division of NRD or a Malaysian consulate in a foreign country, together with two copies of Form K (original) that have been completed.

1. Copy of the foreign passport

2. Copy of the foreign citizenship certificate or copy of confirmation that the applicant will become the citizen of a foreign country

3. Birth Certificate or latest extract of Birth Certificate for applicants born in Malaysia, or Form W for applicants born in a foreign country (if any)

4. Citizenship Certificate (if any)

5. Identity card (if any)

6. 2 Passport-sized photograph of the applicant with a blue background

7. The applicant’s left and right thumbprints

8. Form K must be signed and certified before a magistrate / Justice of Peace / solicitor / notary public or commissioner for oaths

9. Malaysian passport or other travel documents

10. Marriage document (for female applicants below age 21)

Application For Citizenship Under Article 19 Of The Federal Constitution – Aged 21 Years And Older

This is application for Malaysian citizenship by naturalisation.

1. The applicant shall appear before the Registrar of Citizenship when submitting the application.

2. The applicant must be aged 21 years and above on the date of the application.

3. The applicant has resided in the federation for a period of not less than 10 years in a period of 12 years, including the 12 months immediately preceding the date of application.

4. The applicant intends to reside permanently in the federation.

5. The applicant is of good character.

6. The applicant has adequate knowledge of the Malay language.

7. The applicant must be sponsored by two referees who are citizens aged 21 years and above and who are not relatives, not hired persons, and not advocates or solicitors to the applicant.

8. Form C must be completed and submitted together with copies of the necessary documents.

The applicant is required to go to the counter at the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters/branch, bringing along originals and copies of required documents.

Application For Citizenship Under Article 15(2) Of The Federal Constitution – Child Under The Age Of 21 Whose Parent Is A Citizen

This is application for Malaysian citizenship by a person under the age of twenty-one whose parent or parents are (or were at the time of death) citizens.

1. The applicant and the person to be registered shall appear before the Registrar of Citizenship when submitting the application.

2. The person to be registered shall be below age 21 on the day of application.

3. One of the parents is a Malaysian citizen.

4. Application must be submitted by the parent or guardian.

5. Form B must be completed and submitted together with copies of the necessary documents.

6. Appendix A.15 (2), which is a dual purpose form for declaration of acceptance of and application for citizenship under Article 15(2) of the Federal Constitution, must be completed and submitted to a consulate abroad within one (1) year of the date of birth.

The applicant and the person to be registered are required to go to the counter at the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters/branch, bringing along originals and copies of required documents.

Application For Citizenship Under Article 15 (1) Of The Federal Constitution – Wife Of A Citizen

This is an application by a married woman whose husband is a citizen.

1. The marriage is still in effect.
2. She has resided in the federation throughout the two years preceding the date of application and intends to do so permanently.
3. She is of good character.
4. The marriage was registered under written laws in force in the federation.

1. The applicant and her husband shall appear before the Registrar of Citizenship when submitting the application.

2. Form A must be completed and submitted together with copies of the necessary documents.

The applicant and her husband are required to go to the counter of the Citizenship Division of a nearby NRD office, bringing along originals and copies of required documents.

Application For Citizenship Under Article 15A Of The Federal Constitution – Citizenship Registration Under Special Circumstances

Application to register as a Malaysian citizen by persons below age 21 under special circumstances.

  1. The applicant and the person to be registered shall appear before the Registrar of Citizenship when submitting the application.
  2. The person to be registered must be below age 21 on the date of application.
  3. State reason for applying for citizenship under special circumstances in section F and paragraph A of the application form. Include an attachment if the space provided is insufficient.
  4. Form B must be completed and submitted together with copies of the necessary documents.
  5. The application must be submitted by a parent or a guardian who is a relative.

The applicant and the person to be registered are required to go to the counter at the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters/branch, bringing along originals and copies of required documents.

Application For Status Of Citizen Under Article 16 Of The Federal Constitution – Persons Born In The Federation Before Independence Day, 31 August 1957

This is application for registration as a Malaysian citizen by a person aged 18 years and above who was born in the federation before Independence Day.

1. The applicant shall appear before the Registrar of Citizenship when submitting the application.

2. The applicant must have been born in the federation (excluding Sabah and Sarawak) before Independence Day.

3. The applicant must be 18 years and above on the date of application.

4. The applicant has resided in the federation during the 7 years immediately preceding the date of the application, for a total period of at least 5 years.

5. The applicant intends to reside permanently in the federation.

6. The applicant is of good character.

7. The applicant has elementary knowledge of the Malay language.

8. Form A must be completed and submitted together with copies of the necessary documents.

The applicant is required to go to the counter of the Citizenship Division of the Putrajaya NRD headquarters, state NRD headquarters/branch, bringing along originals and copies of required documents.

Application For Search Of The Citizenship Register

Any person who wishes to obtain a search on an application form, certificate, notice, order or declaration from the register as provided for under Part III of the Constitution or under the provisions of the Federation of Malaya Agreement, 1948 may do so by making a written application using the prescribed form.

Form O that is completed together with A4 copies of necessary documents

Applications may be submitted at any counter at the Citizenship Division of the Putrajaya NRD headquarters, state NRD headquarters/branches or by post.

1. Information required for the search (such as certificate number, subject, place and date of issue)

2. Applicant’s identity card number (also attach the Birth Certificate if the applicant’s name is recorded in the parents’ Citizenship Certificates)

3. Provide clear reasons in the application form for requesting the search, stating the year to search for the Citizenship Certificate.

4. The applicant shall be the:

4.3. Spouse of document owner 
4.4. Child of document owner 
4.5. Next of kin (grandfather/grandmother only)

1. RM5.00 fee to search through a period of one year or part thereof

Any person who wishes to obtain an extract or certified copy of the application form, certificate, notice, order or declaration from the register as provided for under Part III of the Constitution or under the provisions of the Federation of Malaya Agreement, 1948 may do so by making a written application using the prescribed form.

3. If the applicant is the child of the Citizenship Certificate owner and the certificate owner has died, then the Death Certificate of the parent must be attached.
