Application For Replacement Of Citizenship Certificate

Any person who wishes to obtain a replacement of the Citizenship Certificate that was lost, damaged or destroyed may do so by making a written application using the prescribed form.

Form P must be completed and submitted together with A4 copies of the necessary documents.

Applications may be submitted at the counter of the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters/branches or by post.

1. Information required to issue a replacement (such as certificate number, subject, place and date of issue)

2. Applicant’s identity card or extract of details from the identity card register

3. Citizenship Certificate (if any)

4. 4 Recent passport-sized photograph of the document owner with a blue background

5. Only copies of documents need to be sent for applications made through the post

6. The applicant shall be the:

6.1. Document owner or
6.2. Parent of the document owner or 
6.3. Spouse of the document owner or
6.4. Child of the document owner or 
6.5. Next of kin (grandfather/grandmother only)

1. RM2.00 for application fee 
2. RM5.00 to issue the document

Note: Payment is by cash or postal order of RM2.00 and RM5.00 values.

All applications for citizenship matters require the use of original forms. Forms may be obtained from the Putrajaya NRD headquarters or state NRD headquarters/ branches. Application forms for registration of birth in a foreign country and renunciation of citizenship may be obtained from consulates abroad.

Application For Citizenship Under Article 14 Of The Federal Constitution – Confirmation Of Citizenship (Born outside Malaysia)

This is an application to confirm the status of a person born outside Malaysia on or after Malaysia Day (16 September 1963) based on the status of the father.

1. The applicant and the person to be registered shall appear before the Registrar of Citizenship/consular officer when submitting the application in accordance with Rule 28 of the Citizenship Rules 1964 (Amendment 1996).

2. The applicant shall be the:

2.1. parent;
2.2. guardian or next of kin.

3. Form D must be completed and submitted together with copies of the necessary documents.

The applicant and the person to be registered are required to go to the counter at the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters or Malaysian consulate in a foreign country, bringing along originals and copies of required documents.

1. REGISTRANT’S DOCUMENTS (ORIGINALS AND COPIES)

1.1. Birth Certificate or latest extract of Birth Certificate (translated into Malay or English by the court/consular office/public translator);
1.2. Entry permit or passport or other travel document;
1.3 Three (3) recent passport-sized photographs with a blue background

2. PARENTS’ DOCUMENTS (ORIGINALS AND COPIES)

2.1 Birth Certificate or latest extract of Birth Certificate
2.2 Identity card (if any)
2.3 Passport or other travel document
2.4 Citizenship Certificate
2.5 Marriage document

Note: All copies of supporting documents should be made on A4 paper.

1. RM20.00 for application fee

2. RM10.00 for issue of the certificate by the consulate (if approved)

3. RM30.00 for issue of the certificate by the Registrar-General of Citizenship with the approval of the Minister of Home Affairs

Note: Payment with postal order is accepted in multiples of RM10.00.

2. Where a child is born before the parents’ marriage is registered, the child’s status follows that of the mother as stated in Section 17, Part III of the Second Schedule of the Federal Constitution.

3. Form E must be completed and submitted together with copies of the necessary documents.

The applicant and the person to be registered are required to go to the counter at the Citizenship Division of the Putrajaya NRD headquarters or state NRD headquarters or NRD branch, bringing along originals and copies of required documents.

1. REGISTRANT’S DOCUMENTS (ORIGINALS AND COPIES)

1.1 Birth Certificate or latest extract of Birth Certificate
1.2 Identity card (if any)
1.3 Passport or other travel document (if any)
1.4 Documentary evidence of schooling (if applicable)
1.5 Three (3) recent passport-sized photographs with a blue background

2. PARENTS’ DOCUMENTS (ORIGINALS AND COPIES)

2.1 Birth Certificate or latest extract of Birth Certificate (if any)
2.2 Identity card (if any)
2.3 Entry permit or passport or other travel document (if any)
2.4 Citizenship Certificate (if any)
2.5 Marriage document (Muslim couples who married in a foreign country must submit confirmation by the Islamic Religious Department)

3. SIBLING’S DOCUMENTS (ORIGINALS AND COPIES)

3.1 Birth Certificate or latest extract of Birth Certificate
3.2 Identity card (if any)

Note: All copies of supporting documents should be made on A4 paper. 

1. RM2.00 for application fee

2. RM10.00 to issue Status Verification Certificate (if approved)

Note:Payment with postal order is accepted in multiples of RM10.00.
