Application To Correct Clerical Errors [Section 27 (2)] In The Death Certificate (Peninsular)


Reason For Application

Application to correct spelling errors which even if not corrected will not change the facts in the Death Certificate. Application can be made at any NRD office in Peninsular Malaysia.

Application Condition

Application to correct information in the death certificate may be submitted by the following persons:

  1. Parent of the deceased;
  2. Spouse of the deceased;
  3. Sibling of the deceased;
  4. Child or legally adopted child of the deceased
Required Documents
  1. Forms JPN.LM21 and JPN.LM22 that are completed;
  2. Death Certificate of the deceased (original and copy);
  3. Identity card or identification document of the applicant (original and copy);
  4. Passport or insurance documentation or bank book of the deceased if the deceased had a passport or insurance or bank account (original and copy);
  5. Marriage certificate or document if application is made by the spouse;
  6. Other supporting documents

Application fee: No charge
