Death


One is father/mother/relative has passed away in Makkah while performing the hajj. What is the procedure for the registration of death?

The death of a Malaysian pilgrim in Makkah would have been registered by the appointed Hejaz Registrar during the pilgrimage period.  An extract of the death certificate will be obtained after the applicant (legitimate beneficiary) fills in the Form JPN.LMH4 and attachs the original or copy of the applicant’s identity card to be submitted to the NRD Headquarters in Putrajaya.

For cases other than the above, the applicant is advised to register the death within 6 months of the date of death to the NRD Headquarters in Putrajaya by submitting documents belonging to the deceased such as certified copies of the passport, identity card, birth certificate (if any), letter certifiying death by the Government of Saudi Arabia, burial permit, flight ticket and other supporting documents.
