Application For Re-Registration Of Birth (Lost Or Damaged) Section 4A (Peninsular)
Application for re-registration of birth registration is made when the register or record of birth cannot be traced due to loss or damage. Applications may be submitted at any NRD office in Peninsular Malaysia, supported by evidence that the birth was registered.
An application for re-registration of birth can be made by the following individuals:
- The child’s mother or father.
- The owner of the birth certificate if they already have an identity card.
Applications can be made at nearby NRDs throughout Peninsular Malaysia.
- JPN.LM14 Search Result (for births occurring in 2002 and below) (original and copy).
- JPN.LM01 Birth Registration Form which has been filled in completely (original).
- Birth Certificate (original and copy) – if available.
- Identity Card of the owner of the Birth Certificate (original and copy) – if available.
- BMK81 Family Information Form (original).
- School Register Extract (original and copy) – if available.
No fee is charged.
