Application For Re-Registration Of Legitimated Person (Section 17) (Peninsular)

Application for re-registration of birth for children listed in Section 13 of the Birth Certificate (Non-Muslim) after obtaining a Legitimacy Order from the High Court.

An application for re-registration of a Notarized Person can be made by the child’s mother or father.

Legal registration can be made at NRDs near all over Peninsular Malaysia.

1. Birth Registration Form JPN.LM01 which has been completed completely (original).
2. Child’s Birth Certificate (original).
3. Child’s Identity Card (original and copy) – if available.
4. Siblings’ Birth Certificate (original and copy) – if available.
5. Mother’s and father’s identity cards (original and copy).
6. Parents’ Marriage Document (original and copy).
7. Court Order that the child has been legalized (original and copy).
8. Family Information Form BMK81 (original).

1. No fee is charged if the registration application is made within 3 months from the date of the parents’ marriage.
2. An application fee of RM 50.00 is charged if the registration application is made after three (3) months from the date of the parents’ marriage.
