Application For Re-Registration Of Legitimated Person (Section 17) (Peninsular)
Application for re-registration of birth for children listed in Section 13 of the Birth Certificate (Non-Muslim) after obtaining a Legitimacy Order from the High Court.
An application for re-registration of a Notarized Person can be made by the child’s mother or father.
Legal registration can be made at NRDs near all over Peninsular Malaysia.
- Birth Registration Form JPN.LM01 which has been completed completely (original).
- Child’s Birth Certificate (original).
- Child’s Identity Card (original and copy) – if available.
- Siblings’ Birth Certificate (original and copy) – if available.
- Mother’s and father’s identity cards (original and copy).
- Parents’ Marriage Document (original and copy).
- Court Order that the child has been legalized (original and copy).
- Family Information Form BMK81 (original).
- No fee is charged if the registration application is made within 3 months from the date of the parents’ marriage.
- An application fee of RM 50.00 is charged if the registration application is made after three (3) months from the date of the parents’ marriage.
