Application To Change Name [Section 15(1)] In The Birth Certificate For Child Under 1 Year (Peninsular)

Application for change of name of a child that is already registered must be made within one (1) year of the date of birth of the child

An application to change a child’s name can be made by the following individuals:

1. The child’s biological mother or father.
2. Legal guardian (attached with child custody authorization letter from JKM).

Applicants are required to appear at the nearest NRD throughout Peninsular Malaysia.

CHILD’S DOCUMENTS (ORIGINALS AND COPIES)

1. Form JPN.LM21 (original).
2. Child’s Birth Certificate (original).
3. Mother and father’s Identity Card (original and copy). Declaration Form (AM80) of mother and father (original).
4. Family Information Form (BMK 81) (original).
5. Parents’ Marriage Document – ​​if any.
