Application To Correct Clerical Errors [Section 27(2)] In The Birth Certificate (Peninsular)
Application to correct minor errors made by the NRD in spelling which even if not corrected will not change the facts in the Birth Certificate
Application to correct the clerical errors may be submitted by the following persons:
- The child’s biological mother or father.
- Legal guardian (attached with child custody authorization letter from JKM).
Applicants are required to appear at the nearest NRD throughout Peninsular Malaysia.
- Form JPN.LM21 (original).
- Form AM 80 – Affidavit (original).
- Child’s Birth Certificate (original).
- Parent’s and Father’s Identity Cards (original and copy).
- Family Information Form (BMK81) (original).
- Parents’ Marriage Document (original and copy) – if available.
- Child’s Siblings’ Birth Certificate or Identity Cards (original and copy) – if available.
No fee is charged.
- JPN.LM21 – Application for Name Entry/Correction of Information in Birth Register
