Application To Correct Factual Errors [Section 27(3)] In The Birth Certificate (Peninsular)

Application to correct any factual errors provided during the registration of birth due to incorrect or unavailable information. The applicant must submit supporting documents proving the factual errors.

Application to correct factual errors may be submitted by the following persons:

1. Owner of the Birth Certificate.
2. The child’s biological mother or father.
3. Legal guardian (attached with a letter of permission for child custody from JKM).

Applicants are required to appear at the nearest NRD throughout Peninsular Malaysia.

1. JPN.LM21 Form (original).
2. Child’s Birth Certificate (original).
3. Parents’ Identity Cards (original and copy).
4. Declaration Form (AM80) of parents (original).
5. Family Information Form (BMK81) (original).
6. Parents’ Marriage Document – (original and copy).
7. Child’s Birth Certificate or Identity Card (original and copy) – if applicable.
8. Other documents that can support the application such as Certificate or Islamic Conversion Card of the child’s parents (original and copy) – if applicable.
9. Child’s Birth Certificate or Death Certificate of the child’s parents (original and copy) – if applicable.
10. Name change approval letter from the Identity Card Division, JPN (original and copy) – if applicable.
11. Letter of consent from the child’s biological parents giving permission to the guardian to make the application – if applicable.
12. Medical Report and Gender Reassignment Technical Committee Report from the Government Hospital (original and copy) – if applicable.
13. Court Order (original and copy) – if applicable.
