Application To Insert Name [Section 15(2)(B)] Into The Birth Certificate For Child Aged 21 And Above (Peninsular)
Application to insert the name of a child aged more than 21 years who was registered without a name at birth.
Application to insert the name for a child over age 21 may be submitted by the following persons:
- Owner of Birth Certificate.
- Biological mother or father.
- Legal guardian (attached with child custody authorization letter from JKM).
The applicant is required to go to any National Registration Department counter in Peninsular Malaysia with the necessary documents.
- JPN.LM21 Form (original).
- Individual Birth Certificate (original).
- Identity Card of parents (original and copy).
- Declaration Form (AM80) of parents (original).
- Family Information Form (BMK81) (original).
- Marriage Document of parents – if any.
Application fee RM 50.00

- BMK 81-Family Information Form
