Extract Of Birth Certificate (Overseas) (Peninsular)

Application to obtain a search/extract of birth register by qualified applicants with the prescribed fee from Overseas.

Applications for birth register search/extraction can be made by the following categories of individuals:

1. Birth Certificate Holders who already have an Identity Card or Passport.
2. Parents (including adoptive parents or stepparents)
3. Legal husband or wife.
4. Children (including adopted children or stepchildren).
5. Siblings (including adopted siblings).

Applicants are required to appear at the nearest Malaysian Embassy in the country concerned.

1. Form JPN.LM12 (born in the Peninsula)/ Form N1 (born in Sabah)/Form X (born in Sarawak) which has been filled in completely (original).
2. Applicant’s Identity Card or personal identification document (original and copy).
3. Applicant’s Birth Certificate (original and copy – if available)
4. Cover letter from the Malaysian Representative Office/Malaysian Embassy.
5. Applicant’s passport (copy).
6. Birth certificate extraction payment receipt amounting to RM5.00 / application payment receipt (Birth Register Search Result) amounting to RM10.00.
7. The Malaysian Representative Office / Embassy must verify the copy of the submitted document before sending the document to the Putrajaya NRD for processing.
