Extract Of Birth Certificate (Peninsular)

Application by an eligible applicant to obtain a copy of the birth register by paying a prescribed fee.

Applications for search / extraction of birth records can be made by the following categories of individuals:

1. Birth Certificate holders who already have an Identity Card or Passport.
2. Parents (including legal adoptive parents).
3. Legal husband or wife.
4. Children (including adopted children or stepchildren of the same mother/father).
5. Siblings (including biological siblings/adoptive siblings/half-siblings).
6. Grandparents of the same mother/father.
7. Biological grandchildren.
8. Legal guardians (attached with a letter of permission to care for the child from JKM (Form 13) / Court Order.
9. Law Firms (representing private agencies/GLCs/Financial Institutions/other individuals not related to the document owner).
10. Statutory Bodies (agencies must apply through their respective lawyers/gentlemen).
11. Government Agencies.

Extract of a birth certificate may be made at any NRD office in Peninsular Malaysia.

A. INDIVIDUALS IN CATEGORY 1 TO 8

1. Form JPN.LM12 (born in the Peninsula)/ Form N1 (born in Sabah)/ Form X (born in Sarawak) which has been completely filled in (original).
2. Applicant’s Identity Card or identification document (original and copy).

B. INDIVIDUALS IN CATEGORY 9

1. Form JPN.LM12 (born in the Peninsula)/ Form N1 (born in Sabah)/ Form X (born in Sarawak) which has been completely filled in (original).
2. Official letter from the Law Firm Office to apply for a search / extract in the death register (original).
3. Letter of appointment from the client (original).
4. Lawyer’s Identity Card or identification document (original and copy).
5. Client’s Identity Card or identification document (copy certified by the Law Firm Office).
6. Letter of power of attorney to manage the applicant’s application and identification document (original and copy – if applicable).
7. Valid Legal Practice Certificate (original/copy certified by another lawyer.

C. INDIVIDUALS IN CATEGORY 10 AND 11

1. Form JPN.LM12 (born in Peninsular Malaysia)/ Form N1 (born in Sabah)/ Form X (born in Sarawak) which has been completely filled in (original).
2. Official Letter from Statutory Body or Government Agency to apply for search / extraction in death register (original).
3. Letter of power of attorney to manage the application and applicant’s identification documents (original and copy – if relevant).
