Extract Of Birth Certificate (Sabah)

Application by an eligible applicant to obtain a copy of the Birth Certificate by paying a prescribed fee. Extract of the Birth Certificate can be made only at the State NRD Office and Putrajaya NRD.

1. Birth Certificate owner;
2. Parent of Birth Certificate owner;
3. Spouse of Birth Certificate owner;
4. Child or legally adopted child of Birth Certificate owner;
5. Sibling of Birth Certificate owner;
6. Law firm;
7. Statutory body;
8. Government agency

CATEGORY 1: PERSON IN CATEGORIES 1 TO 5

1. Form N1 that is completed;
2. Identity card or identification document (original and copy);
3. Copy of Birth Certificate of owner (if available)

CATEGORY 2: PERSON IN CATEGORY 6

• Form N1 that is completed;
• Official letter from a law firm requesting search or extract of the register;
• Letter of appointment from client (applicant);
• Identity card or identification document of client (applicant) (original and copy);
• Power of attorney to oversee the application (if applicable)

CATEGORY 3: PERSON IN CATEGORIES 7 AND 8

• Form N1 that is completed;
• Official letter from a statutory body or government agency to apply for a search or extract of the register;
• Power of attorney to oversee the application (if applicable)

1. Application fee is RM5.00.
2. Government agencies are exempted from payment.
