Extract Of Certificate Of Birth In Hedjaz (Peninsular)

Application to obtain a search/extraction of birth registers by qualified applicants with the prescribed fee.

Application for extract of Birth Certificate may be submitted by the following persons or entities:

1. Birth Certificate owner who currently holds an identity card;
2. Parent of Birth Certificate owner;
3. Spouse of Birth Certificate owner;
4. Child or legally adopted child of Birth Certificate owner;
5. Sibling of Birth Certificate owner;
6. Legal guardian of the child (birth certificate owner) has a letter of custody of children under the Child Protection Act 1991;
7. Law firm;
8. Statutory body;
9. Government agency

Applicants are required to appear at the Putrajaya NRD Headquarters or State NRD only.

CATEGORY 1: PERSON IN CATEGORIES 1 TO 6

1. Form JPN.LMH3 that is completed;
2. Identity card or identification document (original document and one (1) copy of the original)

CATEGORY 2: PERSON IN CATEGORY 7

1. Form JPN.LMH3 that is completed;
2. Official letter from a law firm requesting search or extract of the register;
3. Letter of appointment from client (applicant);
4. Identity card or identification document of client (applicant) (one (1) copy of original document);
5. Power of attorney to oversee the application (if applicable)

CATEGORY 3: PERSON IN CATEGORIES 8 AND 9

1. Form JPN.LMH3 that is completed;
2. Official letter from a statutory body or government agency to apply for search or extract of the register;
3. Power of attorney to oversee the application (if applicable)

Application fee is RM2.00. Government agencies are exempted from payment.
