Late Registration Of Birth (Peninsular)

Late registration of birth refers to registration made more than 60 days after the date of birth of a child in Malaysia.

Late birth registration applications must consist of the following categories of individuals:

1. The child’s mother and father.
2. The mother or father and an organizer who is aware of the child’s birth. The organizer’s age is 10 years older than the child (if the mother or father cannot be traced / deceased).
3. Two (2) organizers who are aware of the child’s birth. The age of each organizer is 18 years and above and the age difference is at least 10 years older than the child (if the mother or father cannot be traced / deceased).
   
   IF APPLICATION IS VIA THE SOCIAL WELFARE DEPARTMENT:

1. One (1) organizer who is a Social Welfare Officer and one (1) individual who is given custody of the child in accordance with the Child Protection Act 1991 (if the child is found vulnerable).

Application for late registration of birth can be made throughout Peninsular Malaysia by bringing along the necessary documents to any NRD office near the applicant’s place of residence.

1. JPN.LM14 Search Result (for births occurring in 2002 and below) (original and copy).
2. JPN.LM01 Birth Registration Form that has been completed completely (original).
3. Birth Confirmation Form from the hospital or Midwife / Doctor Confirmation Letter for home birth (original).
4. Police Report if the birth occurred at home and the child’s biological parents cannot be traced (original).

CHILDREN’S DOCUMENTS

1. 1 recent full-length photograph of the child, size 4R.
2. 1 recent passport-sized photograph of the child with a blue background.
3. Child Health & Immunization Card (original and copy).
4. School Register Extract or School Certificate with confirmation by the Headmaster or School Principal if the child is in school and is seven (7) years old or above or Letter of Declaration of Child Never Having Been to School filled in by the Headman or Tribal Head or Member of Parliament or Assemblyman in the area where the child lives and the child is seven (7) years old or above (original and copy).
5. Letter of Declaration of Child Birth by the Village Head or Tribal Head or Member of Parliament or Assemblyman in the area where the child was born – for births that occurred at home (original).

PARENTS’ AND FATHERS’ DOCUMENTS

1. Identity Card or Passport or Birth Certificate or Death Certificate (original and copy).
2. Marriage or Divorce Document (original and copy).
3. 1 recent passport-sized photograph of the parents with a blue background.
4. Prenatal Card (Pregnancy Treatment Book) or Pregnancy Treatment Card (original and copy).

SIBLINGS DOCUMENTS

1. Birth Certificate and Identity Card of the child’s siblings (copy).
2. 1 recent passport-sized photograph of the siblings with a blue background.
3. Family Information Form BMK81 and BMK81A (original).

ORGANIZER’S DOCUMENTS

1. Identity Card or Passport or UNHCR Card (original and copy).
2. Declaration Form AM 80 for the organizer who knows about the child’s birth (original).

WITNESS DOCUMENTS

1. Identity Card or Passport or UNHCR Card (original and copy).
2. Declaration Letter of the Witness who knows about the child’s birth – if the birth occurred at home (original).

ADDITIONAL DOCUMENTS

1. Declaration Letter (SA81) (original).
2. Utility Bill (water bill / electricity bill / astro bill / telephone bill) – If the residential address is different from the address of the identity card / passport holder and non-citizen documents.

CHILD FOUND EXPOSED (ADOPTED CHILD / CUSTODY CHILD)

1. Police Report – if the biological mother cannot be traced (original and copy).
2. Temporary Custody Rights From the Social Welfare Department (JKM) – if any (original and copy).
3. Juvenile Court Order – if relevant (original and copy).
