Regulation 12 (1) and (2) of the National Registration Regulation 1990 (Amendment 2001) provides that, the register shall not be opened for public inspection. No public officer, unless duly authorized by the Director-General or a police officer in the exercise of his power under Chapter XIII of the Criminal Procedure Code, may inspect the register; but extracts from the register may be issued to public officers at the discretion of Director-General or payment of a fee as prescribed by these Regulations to any member of the public.

1. Extract of Register – Identity Card (JPN.KP12)
   
   • Applicant / beneficiary / representative  is required to go to the  NRD Counter or mail the application to the NRD headquarters in Putrajaya or the State NRD headquarters.
     
     
2. Extracts of Register – Address (JPN.KP13)
   
   • Applicant or representative (government agencies/ law enforcement agencies/ statutory bodies/legal firm) is required to present detailed information to JPN office through counter/official e-mails/letter with the organization letter  for further review.
   • Applications letters submitted by applicants must have official letterhead.
   • Application any email need to be address as the Appendix 

1. Extracts of Register – Identity Card (JPN.KP12)
2. The documents submitted by the owner are as follows :
   
   • Identity card (original and copy); or
   • Entry Permit; or
   • Other documents such as birth certificate, death certificate, driving license, passport, bank account books or other documents which recorded identity card number.
     
     
3. Documents submitted by beneficiaries are as follows:
   
   • Applicant identity card; or
   • Birth Certificate of the applicant and the owner ; or
   • Death Certificate ; or
   • Marriage Certificate ; or
   • Other documents to proof the relationship with the owner.
4. Extracts of Register – Address (JPN.KP13)
5. Documents submitted by representatives / Government agencies / Law enforcement agencies / Statutory Agencies / Legal firms.
   
   • Letter from legal firm representing the third party /representatives
   • Application letter that bear the letterhead of a government agencies /Law enforcement agencies / statutory bodies / Legal firms.
