Late Registration Of Birth (Peninsular)

Reason For Application

Late registration of birth refers to registration made more than 60 days after the date of birth of a child in Malaysia.

Application Conditions

APPLICATION CONDITIONS ELIGIBILITY OF THE APPLICANT, SPONSOR OR INFORMANT

The applicants shall consist of two (2) persons, namely:

  1. Parents;
  2. Mother or father and a sponsor with knowledge of the birth who is 15 years older than the child (if one of the parents cannot be found / is  deceased);
  3. Two (2) sponsors acquainted with the child, who are both aged 21 and above and are at least 15 years older than the child (if both parents cannot be found / are deceased);
  4. Two (2) sponsors who are social welfare officers or two (2) persons who have been given custody of the child in accordance with the Child Protection Act 1991 (if the child is found stranded)
Application Procedure

Application for late registration of birth can be made throughout Peninsular Malaysia by bringing along the necessary documents to any NRD office near the applicant’s place of residence.

Required Documents (Home Birth)

CHILD’S DOCUMENTS (ORIGINALS AND COPIES)

  1. Birth registration search form JPN.LM12 (for births from 2002 onwards);
  2. Birth registration form JPN.LM01 that is completed;
  3. Prenatal card or maternity examination book, child’s health and immunisation card, certification of home birth by midwife/doctor (originals and copies);
  4. Recent full-length 4R photograph (one (1));
  5. Recent passport-sized photograph (one (1));
  6. Certification of birth from a headman or village chief or Member of Parliament or member of a State Legislative Assembly for the constituency where the child was born;
  7. Extract of school register or school certificate with endorsement from the head teacher or school principal if the child is schooling and aged seven (7) and above, or alternatively, certification from a headman or village chief or Member of Parliament or member of a State Legislative Assembly for the constituency where the child resides, that the child has never been schooled and is aged seven (7) and above.
  8. Statutory declarations of two (2) witnesses with knowledge of the birth

PARENTS’ DOCUMENTS (ORIGINALS AND COPIES)

  1. Identity cards or entry permits or passports or death certificates (originals and copies);
  2. Marriage certificates or divorce certificate (originals and copies);
  3. Recent passport-sized photograph of each parent (one (1))

SIBLING’S DOCUMENTS (ORIGINALS AND COPIES)

  1. Birth certificate or identity card, if any (original and copy);
  2. Recent passport-sized photograph of the sibling from the same parents (one (1));
  3. Family information forms BMK81 and BMK81A

SPONSOR’S DOCUMENTS (ORIGINALS AND COPIES)

  1. Statutory declaration JPN.LM27 for two (2) sponsors with knowledge of the birth;
  2. Identity card or entry permit or passport or death certificate of the informant, witness or sponsor (original and copy)
Required Documents (Birth In Government/Private Hospital or Clinic)

 CHILD’S DOCUMENTS (ORIGINALS AND COPIES)

  1. Birth registration search form JPN.LM12 (for births from 2002 onwards);
  2. Birth registration form JPN.LM01 that is completed;
  3. Confirmation of birth form;
  4. Prenatal card or maternity examination book, child’s health and immunisation card, certification of home birth by midwife/doctor (originals and copies);
  5. Recent full-length 4R photograph (one (1));
  6. Recent passport-sized photograph (one (1));
  7. Extract of school register or school certificate with endorsement from the head teacher or school principal if the child is schooling and aged seven (7) and above, or alternatively, certification from a headman or village chief or Member of Parliament or member of a State Legislative Assembly for the constituency where the child resides, that the child has never been schooled and is aged seven (7) and above.

  2. Recent passport-sized photograph of the sibling from the same parents (one (1));
  3. Family information forms BMK81 and BMK81A

SPONSOR’S DOCUMENTS (ORIGINALS AND COPIES)

  1. Statutory declarations JPN.LM27 of two (2) sponsors with knowledge of the birth;
  2. Identity card or entry permit or passport or death certificate of the informant or witness or sponsor (original and copy)
