Registration Of Stillbirth (Sabah)

Application Procedure

The applicant must complete Form A and Form N1(Extract) and attach supporting documents.

Required Documents (Original and Copies)
  • Personal identification document or a valid travel document
  • Pre-natal examination document.
  • Confirmation of stillbirth
  • Marriage document of the parents (for firstborn only) – if married.
