Application To Correct Clerical Errors [Section 30] In The Birth Certificate (Sabah)

Reason For Application

Application to correct minor errors made by the Registrar involving spelling errors only which even if not corrected will not change the facts in the Birth Certificate.

Application Conditions

Application to correct the clerical errors may be submitted by the following persons:

 

  1. Parent;
  2. Legal guardian

 

 

Application Procedure

The applicant is required to go to any National Registration Department counter in Sabah with the required documents.

Required Documents (Original and Copies)

CHILD’S DOCUMENTS (ORIGINALS AND COPIES)

 

  1. Forms PSB 4 and PSB 4A that are completed;
  2. Birth Certificate (original and copy);
  3. Identity card or identification document (original and copy);
  4. Passport or insurance documentation or bank book if the child has a passport or insurance or bank account (original and copy);
  5. Any other supporting documents

 

 

PARENTS’ DOCUMENTS (ORIGINALS AND COPIES)

  1. Identity card or other identification document (original and copy);
  2. Authorisation letter from the parents for the guardian to submit the application
Payment
  1. Registration fee: RM2.00
  2. Approval fee: RM5.00
  3. Birth Certificate extract fee: RM5.00
