This is application for registration as a Malaysian citizen by a person aged 18 years and above who was born in the federation before Independence Day.

1. Pemohon hendaklah hadir di hadapan Pendaftar Warganegara semasa mengemukakan permohonan.
2. Pemohon hendaklah dilahirkan dalam Persekutuan sebelum Hari Merdeka (tidak termasuk Sabah dan Sarawak).
3. Pemohon hendaklah berumur 18 tahun ke atas pada tarikh permohonan.
4. Pemohon telah bermastautin dalam Persekutuan dalam tempoh 7 tahun terdahulu dan tidak kurang daripada 5 tahun pada tarikh permohonan dikemukakan.
5. Pemohon berniat untuk bermastautin dalam Persekutuan
6. Pemohon berkelakuan baik.
7. Pemohon mempunyai pengetahuan asas Bahasa Melayu.
8. Mengisi Borang A dengan lengkap dan sertakan salinan dokumen yang perlu dibawa bersama.

The applicant is required to go to the counter of the Citizenship Division of the Putrajaya NRD headquarters, state NRD headquarters/branch, bringing along originals and copies of required documents.

   • Birth Certificate or latest extract of Birth Certificate 
   • Passport or other travel document (if any) 
   • Marriage document (if any) 
   • Three (3) recent passport-sized photographs with a blue background

3. SPOUSE’S DOCUMENT (COPY)

   • Birth Certificate or latest extract of Birth Certificate (if any) 
   • Identity card (if any)

1. RM10.00 for application fee
2. RM100.00 to issue Citizenship Certificate (if approved)

All applications for citizenship matters require the use of original forms. Forms may be obtained from the Putrajaya NRD headquarters or state NRD headquarters/ branches. Application forms for registration of birth in a foreign country and renunciation of citizenship may be obtained from consulates abroad.
