An application for search is to obtain the result from the register of death on payment of the prescribed fee. The application can be made only at the State NRD Office and Putrajaya NRD.

Application for search of Death Certificate may be submitted by the following persons or entities:

1. Parent of the deceased;
2. Spouse of the deceased;
3. Child or legally adopted child of the owner of the Death Certificate;
4. Sibling of the owner of the death certificate;
5. Government agency;
6. Statutory body;
7. Law firm

CATEGORY 1: PERSON IN CATEGORIES 1 TO 4

1. Form JPN.LM13 that is completed;
2. Identity card or identification document of the applicant (original and copy);
3. Copy of Death Certificate (if any)
4. Marriage certificate (original and copied

CATEGORY 2: PERSON IN CATEGORY 5

1. Form JPN.LM13 that is completed;
2. Official letter from a law firm requesting search or extract of the register;
3. Letter of appointment from the client (applicant);
4. Identity card or identification document of the client (applicant) (original and copy);
5. Power of attorney to oversee the application (if applicable)

CATEGORY 3: PERSON IN CATEGORIES 6 AND 7

1. Form JPN.LM13 that is completed;
2. Official letter from a statutory body or government agency to apply for a search or extract of the register;
3. Power of attorney to oversee the application (if applicable)

CATEGORY 4: OTHER THAN PERSON IN CATEGORIES 1 TO 4

1. Form JPN.LM13 that is completed;
2. Identity card or identification document (Non Citizens);
3. Official letter to apply for a search or extract of the register;
4. Statutory Declaration (if applicable);
5. Family Tree (if applicable);
6. Power of attorney to oversee the application (if applicable);
7. Any document or information that can be used as a basis for making a search on a birth register such as a school attendance letter and another siblings’ birth certificate (for a birth search application).

Application fee is RM10.00
