ACTION OF DEATH VERIFICATION AGENCIES (HOSPITALS, POLICE OR KUALA LUMPUR CITY HALL)

1. Receive information regarding a death from an informant;
2. Verify and issue death registration / burial permit JPN.LM02 (Amendment 1/11) and hand the blue copy of the document to the informant;
3. Submit the grey copy of death registration / burial permit JPN.LM02 (Amendment 1/11) to the nearest NRD within 7 working days and the NRD will record information in the I-NRD system. The informant or the next of kin of the deceased may then obtain the Death Certificate from the assigned NRD office.
   
   

DOCUMENTS TO BE PRODUCED BY INFORMANT OR NEXT OF KIN OF THE DECEASED AT THE NRD OFFICE

1. Copy of the blue death registration/burial permit JPN.LM02 (Amendment 1/11);
2. Original identity card or identification document of the deceased;
3. Identity card or identification document of the informant (original and copy);
4. Certification by a medical practitioner of the cause of death [form JPN.LM09 or JPN.LM10 (Post-Mortem)] if death occurred in hospital.
   
   

INDIVIDUALS ELIGIBLE TO COLLECT THE DEATH CERTIFICATE

1. Parent of the deceased;
2.  Spouse of the deceased;
3. Child of the deceased;
4. Sibling of the deceased;
5. Informant of the death as recorded in the death registration/burial permit JPN.LM02 (Amendment 1/11).

ACTION OF NEXT OF KIN OF THE DECEASED OR INFORMANT

Report the death to a death verification agency (hospital or police or Kuala Lumpur City Hall);
Receive a blue copy of the death registration/burial permit JPN.LM02 (Amendment1/11) from the death verification agency;
Submit a copy of JPN.LM02 (Amendment 1/11) to a cemetery caretaker for burial purposes; and
Register at a nearby NRD and collect the death certificate.

DOCUMENTS TO BE PRODUCED BY NEXT OF KIN OF THE DECEASED AT THE NRD OFFICE

Copy of the blue death registration/burial permit JPN.LM02 (Amendment1/11);
Original identity card or identification document of the deceased;
Identity card or identification document of the informant;
Certification by a medical practitioner of the cause of death [form JPN.LM09 or JPN.LM10 (Post-Mortem)] if the deceased died in hospital.
