Every death that occurs during the hajj pilgrimage must be reported to the registrar in Hejaz within 40 days of the date of death or at the counter of the NRD Headquarters within 6 months of the date of death.

The application to register the death of a person in Hejaz can be made at any NRD offices in Peninsular Malaysia by submitting the fully completed Form JPN.LMH2 together supporting documents. However, approval will be given only by the NRD Headquarters.

3. Identification documents of the informant (if the informant is not the parent)
