Frequently asked questions and answers for public reference.

The Birth Certificate is lost, damaged or destroyed. How do I get a Copy or Extract of Birth Certificate?

The Owner of the Birth Certificate/ parents/ heirs may apply for the Extract of the Birth Certificate by:-

Attend the nearest NRD office with your Identity Card or identity document (original and copy) and fill out Form NRD.LM12. A fee of RM 5.00 is charged per Birth Certificate Extract.

A mother who has just given birth to a child. Who can inform the birth of the child?

Individuals who can notify the birth of the child are as follows:-

  • Father of child or;
  • The child's mother or;
  • Persons living in the household where the child was born or;
  • Any person witnessing the birth; or
  • Any person caring for a child.
The child does not have a Birth Certificate, is over 60 days old and was born within Peninsular Malaysia

If a parent or legal guardian has proof that the child's birth actually took place within Peninsular Malaysia, a Late Registration of Birth Application can be made at any nearby NRD counter within Peninsular Malaysia.

Child birth occurs outside of marriage (child born out of wedlock/child born out of wedlock) (for non-Muslims)

Father's information for children born out of wedlock will not be recorded in the birth register, except with a joint application by the mother and the person claiming to be the father of the child. A person claiming to be the father of the child must sign the birth register together with the child's mother.

The parent wishes to change the child's name on the Birth Certificate.

The name of the child on the Birth Certificate may be changed or changed through an Information Correction Request if the child has not attained (1) year of age.
