Application extract birth certificate (adopted child) through court order may be made by the following individuals:

1. Adoptive parents;
2. Siblings angkat;
3. Pemilik Birth Certificate (Adopted Child);
4. Legal Firm that dilantik by client.

Forms NRD.AA04 that telah complete diisi;

1. Identity Card or pasport applicant original and copy);
2. Sekiranya application dikemukakan by Legal Firm, copy Identity Card or pasport client, surat lantikan original and copy Identity Card wakil must be enclosed.
3. Satu (1) copy original Birth Certificate (Adopted Child) child (jika applicant membuat application gantian certificate baru).
