Application to Correct Clerical Error Section 25(2) in Birth Certificate (Sarawak)

This application is to correct minor errors made by NRD involving spelling mistakes where the correction does not alter the facts in the Birth Certificate.

Application to Correct Information Section 25(2) can be made by the following individuals:

• Biological mother or father of the child;
• Legal guardian of the child;

CHILD DOCUMENTS (ORIGINAL AND COPY)

• Form X and Form VII completed;
• Child Birth Certificate (original document and one (1) copy of document original);
• Passport, insurance document or bank book of the child if the child already has a passport, bank account or insurance (original document and one (1) copy of document original);
• other documents that can support the application.
  

PARENTS DOCUMENTS (ORIGINAL AND COPY)

• Identity Card or identification document mother and father (original and  copy);
• Guardianship Consent Letter from Social Welfare Department (if the application is made by the child guardian) (original and  copy );
  

SIBLINGS DOCUMENTS (ORIGINAL AND COPY)

• Birth Certificate or Identity Card of the child siblings, if any (original  and copy);

1. Registration application fee RM2.00.
2. Birth Certificate extract fee RM 5.00.
