Application to Correct Factual Error Section 25(3) in Birth Certificate (Sarawak)

This application is to correct factual errors where information other than the child name provided by the informant during birth registration is incorrect or incomplete. The applicant must submit supporting documents to prove the factual error.

Applicant is required to be present at any National Registration Department counter in Sarawak with the required documents.

Application to Correct Information Section 25(3) can be made by the following individuals:

• Birth Certificate Owner;
• Biological mother or father of the child;
• Legal guardian of the child;

CHILD DOCUMENTS (ORIGINAL AND COPY)

• Form X and Form VII completed;
• Statutory Declaration SA/81;
• Family Information Form BMK-81;
• Child Birth Certificate (original and copy );
• Passport, insurance document or bank book of the child if the child already has a passport, bank account or insurance (original and copy );

PARENTS DOCUMENTS (ORIGINAL AND COPY)

• Identity Card or identification document of the mother and father or Death Certificate of the child parents (original and copy);
• Consent Letter from the biological parents of the child granting permission to the guardian to make the application;
• Marriage Certificate or Divorce Certificate (original and copy);
• Guardianship Consent Letter from Social Welfare Department (if the application is made by the child guardian) (original and  copy );
• other documents that can support the application such as certificate or card of conversion to Islam of the child mother or father, Birth Certificate or Death Certificate of the child mother or father, and approval letter for change of name from the Identity Card Division, NRD;

SIBLINGS DOCUMENTS (ORIGINAL AND COPY)

• Birth Certificate or Identity Card of the child siblings, if any (original and copy);

1. Registration application fee RM2.00;
2. Approval application fee RM 5.00
3. Birth Certificate extract fee RM 5.00.
