Application to Correct Information Section 12 in Birth Certificate for Insertion of Child Name Under One (1) Year (Sarawak)

This application is intended to insert the name of a child who was registered as “No Name” during birth registration and is less than one (1) year old.

Application to Correct Information Section 11 can be made by the following individuals:

1. Birth Certificate Owner;
2. Biological mother or father of the child;
3. Legal guardian of the child;

Applicant is required to be present at any National Registration Department counter in Sarawak with the required documents.

CHILD DOCUMENTS (ORIGINAL AND COPY)

1. Form Form VII and Form X completed;
2. Statutory Declaration;
3. Family Information Form BMK81;
4. Child Birth Certificate (original and copy );
5. Identity Card or identification document child (original and Copy)
6. Passport, insurance document or bank book of the child if the child already has a passport, bank account or insurance (original and copy );
7. School declaration letter if the child is schooling;
8. Penghulu Declaration Letter in the child area of residence;
9. other documents that can support the application.

PARENTS DOCUMENTS (ORIGINAL AND COPY)

1. Identity Card or identification document of the mother and father or Death Certificate of the child parents (original and copy);
2. Consent Letter from the biological parents of the child granting permission to the guardian to make the application;
3. Marriage Certificate or Divorce Certificate (original and copy);
4. Guardianship Consent Letter from Social Welfare Department (if the application is made by the child guardian) (original and  copy );
5. other documents that can support the application such as certificate or card of conversion to Islam of the child mother or father, Birth Certificate or Death Certificate of the child mother or father, and approval letter for change of name from the Identity Card Division, NRD;

1. Birth Certificate or Identity Card of the child siblings, if any (original and copy);

1. Registration application fee RM2.00
2. Approval application fee RM 5.00
3. Birth Certificate extract fee RM 5.00
