Application to Correct Information Section 13 in Birth Certificate for Change of Child Name Under One (1) Year (Sarawak)

Application to change the name of a registered child can only be made within one (1) year from the date of birth of the child.

Application to Correct Information Section 11 can be made by the following individuals:

• Biological mother or father of the child;
• Legal guardian of the child;

Applicant is required to be present at any National Registration Department counter in Sarawak with the required documents.

CHILD DOCUMENTS (ORIGINAL AND COPY)

• Form Form VIl and Form X completed;
• Statutory Declaration;
• Family Information Form BMK81 and BMK 81A;
• Child Birth Certificate; (original and copy);
• Identity Card or identification document applicant; (original and copy);
• Passport, insurance document or bank book of the child if the child already has a passport, bank account or insurance; (original and copy)
• Child photo in 3R postcard size and passport size;
• School declaration letter if the child is schooling;
• Penghulu Declaration Letter in the child area of residence;
  

PARENTS DOCUMENTS (ORIGINAL AND COPY)

• Identity Card or identification document mother and father (original and copy);
• Guardianship Consent Letter from Social Welfare Department (if the application is made by the child guardian);
• Consent Letter from the biological parents of the child granting permission to the guardian to make the application;
