Extract of Birth Certificate (Sarawak)
This application is to obtain a copy of the Birth Certificate by an eligible applicant with the prescribed fee. This birth extract can only be made at the State NRD Office and NRD Putrajaya.
- Birth Certificate Owner;
- Mother or father birth owner Certificate;
- Suami or isteri birth owner Certificate;
- Child biological or child angkat legal for birth owner Certificate;
- Siblings biological birth owner Certificate;
- Law Firm;
- Statutory Body;
- Government Agency.
CATEGORY 1: INDIVIDUALS IN CATEGORIES 1 TO 5
- Form N1 completed;
- Identity Card or identification document applicant (original and copy);
- Copy of the register owner birth certificate (if any).
CATEGORY 2: INDIVIDUALS IN CATEGORY 6
- Form N1 completed;
- Official Letter from Law Firm to apply search and extract in register;
- Surat lantikan from child guam (applicant);
- Identity Card or identification document child guam (applicant) (original and copy);
- Surat wakil kuasa to menguruskan application (if applicable).
CATEGORY 3: INDIVIDUALS IN CATEGORIES 7 AND 8
- Form N1 completed;
- Surat Rasmi Statutory Body or Government Agency to apply search or extract in register;
- Authorization letter from the applicant to manage the application (if applicable).
Fee application RM 5.00. Mchildala application from Government Agency adalah dikecualikan fee.
