Late Registration of Stillbirth (Sarawak)

Normal birth registration means every birth that occurs in Malaysia and is registered within 14 days from the date of birth of the child. The informant must be the mother, father, relative, or a person who knows about the birth.
Section 18, Ordinan Registration Birth & Death 1951 (Sarawak Cap.10)

Applicant must complete Form I and Form X (Extract) with completed together with supporting documents.

1. Document Introduction diri or document perjalanan legal
2. Pregnancy examination document
3. Birth Confirmation or stillbirth
4. Marriage Certificate parents

Fee application : RM 5.00
