Normal Registration of Birth (Peninsular)
Normal registration of birth refers to registration that is made within 60 days of the birth of a child in Malaysia.
Application for registration of a child may be submitted by the following persons:
- Parent;
- Legal guardian;
- Person having knowledge of the birth
- The mother and father of the child (for the registration of births illegitimate child)
The applicant is required to go to any National Registration Department counter with the required documents.
- Birth Registration Form NRD.LM01 which has been filled out completely (original);
- Birth Confirmation Form from the hospital or Doctor’s Confirmation Letter for home birth (original);
- Police Report for home birth (original);
- Prenatal Card (Pregnancy Treatment Book) or Pregnancy Treatment Card (original and copy);
- Identity card or identification document of the mother and father (original and copy)
- Identity card or identification document of the informant [if the informant is not the mother or father of the child] (original and copy);
- Death Certificate of the mother and father of the child (original and copy – if relevant); and
- Marriage or Divorce Document of the child’s parents (original and copy).
- AM80 declaration letter if there is no pregnancy treatment book or pregnancy treatment card.
- Utility Bill (water bill / electricity bill / astro bill / telephone bill) – If the residential address is different from the address of the identity card / passport holder and non-citizen document.
No fee charged.
