Application for Citizenship Status Under Article 14 of the Federal Constitution – Citizenship (Birth in Malaysia)

Application to verify the citizenship status of a person born in Malaysia based on the citizenship status of the mother or father.

1. The applicant and the person being registered must be present before the Citizenship Registrar / Consular Officer when submitting the application in accordance with Rule 28 of the Citizenship Rules 1964 (Amendment 1996).
2. Applicant is:
   – Father or mother ;
   – Guardian or nearest relative.
   – The person to be registered.
3. Complete Form E and enclose copies of the required supporting documents.

The applicant and the registered person are required to appear in person at the Citizenship Division counter—whether at the Putrajaya Headquarters, a state-level NRD (JPN) headquarters, or an NRD branch office—and bring along both the original documents and copies.

1. DOCUMENTS OF THE PERSON BEING REGISTERED (ORIGINAL AND COPY)
   • 1.1 Birth Certificate or recent Birth Certificate extract
   • 1.2 Identity Card (if any)
   • 1.3 Passport or other travel documents (if any)
   • 1.4 Proof of schooling documents (if attending school)
   • 1.5 Three (3) recent passport-sized photographs with a blue background.
     
2. DOCUMENTS OF MOTHER AND FATHER (ORIGINAL AND COPY)
   • Birth Certificate or the latest Birth Certificate extract (if any)
   • Identity Card (if any)
   • Entry permit, passport, or other travel documents (if any)
   • Certificate of Citizenship (if any)
   • Marriage documents (if the marriage took place abroad for a Muslim couple, submit verification from the Islamic Religious Department)
     
3. SIBLING-RELATED DOCUMENTS (ORIGINAL AND COPY)
   • Birth Certificate or Extract Birth Certificate latest
   • Identity Card (if any)
     Note: All copies of supporting documents must be made on A4-sized paper.

1. RM2.00 registration application fee.
2. RM10.00 fee for the issuance of the Status Confirmation Certificate (if approved).

1. All citizenship applications must use the original form. Forms can be obtained from NRD Headquarters, Putrajaya or any State NRD Headquarters/Branch. For application registration birth abroad and renunciation of citizenship, form bby can also be obtained from the Consular Office abroad.
2. For a child born before the marriage is registered, the child status follows the mother status as stated under Section 17, Part III of the Second Schedule of the Federal Constitution.
