Application for Citizenship Status Under Article 14 of the Federal Constitution – Citizenship (Birth in Malaysia)

Application to verify the citizenship status of a person born in Malaysia based on the citizenship status of the mother or father.

1. The applicant and the person being registered must be present before the Citizenship Registrar / Consular Officer when submitting the application in accordance with Rule 28 of the Citizenship Rules 1964 (Amendment 1996).
2. Applicant is:
   – Father or mother ;
   – guardian or nearest relative.
   – the person to be registered.
3. Complete Form E and enclose copies of the required supporting documents.

The applicant and the person being registered are required to be present at the Citizenship Division counter, Headquarters, Putrajaya or each State NRD Headquarters or NRD Cawangan and bring the original documents together with copies.

1. DOCUMENTS OF THE PERSON BEING REGISTERED (ORIGINAL AND COPY)
   • 1.1 Birth Certificate or Extract Birth Certificate terbaru
   • 1.2 Identity Card (if any)
   • 1.3 Pasport or lain-lain documents perjalanan (if any)
   • 1.4 Documents bukti persekolahan (jika bersekolah)
   • 1.5 Tiga (3) keping photograph terbaru berpassport size berlatar belakangkan warna biru.
     
2. DOCUMENTS IBU and BAPA (ORIGINAL AND COPY)
   • Birth Certificate or Extract Birth Certificate terbaru (if any)
   • Identity Card (if any)
   • Permit masuk or pasport or lain-lain documents perjalanan (if any)
   • Certificate Citizen (if any)
   • Documents Marriage (jika marriage berlaku di abroad for pasangan islam, kemukakan pengesahan by Jabatan Agama Islam)Birth Certificate or Extract Birth Certificate terbaru.
     
3. DOCUMENTS ADIK BERADIK (ORIGINAL AND COPY)
   • Birth Certificate or Extract Birth Certificate terbaru
   • Identity Card (if any)
     Nota: Semua copy documents-documents sokongan must dibuat on A4-sized paper. 

1. RM2.00 fees registration application.
2. RM10.00 fees pengeluaran Certificate Pengesahan Taraf (jika is approved).

1. Semua application urusan citizenship must menggunakan form original . Form-form bby didapatkan from NRD Headquarters, Putrajaya or mana-mana State NRD Headquarters/Cawangan. For application registration birth abroad and renunciation citizenship, form bby juga didapatkan dari Consular Office abroad.
2. For a child born before the marriage is registered, the child status follows the mother status as stated under Section 17, Part III of the Second Schedule of the Federal Constitution.

Form Application Pengesahan Taraf Form E (obtain at the NRD counter) has been completed in full.
