Application for Citizenship Status Under Article 15(1) of the Federal Constitution – Wife of a Citizen

This is an application by a married woman whose husband is a citizen.

1. That the marriage is still subsisting.
2. That the person has resided in the Federation throughout the two-year period preceding the date of the application and intends to do so permanently.
3. That the person is of good character.
4. That the marriage has been registered in accordance with any written law in force in the Federation.

1. The applicant and her husband must appear before the Registrar of Citizens when submitting the application.
2. Complete Form A fully and include copies of the required documents.

The applicant and her husband are required to go to the counter of the Citizenship Division of a nearby NRD office, bringing along originals and copies of required documents.

1. APPLICANT’S DOCUMENTS (ORIGINAL AND COPY)
   • Identity card 
   • Birth Certificate or latest extract of Birth Certificate or Adoption Certificate (if any)
   • Marriage document registered in accordance with the written law in Malaysia
   • Entry permit or extract of details from the applicant’s identity card register
   • Passport or other travel document (if any)
   • Three (3) recent passport-sized photographs with a blue background
     
2. HUSBAND’S DOCUMENTS (ORIGINAL AND COPY)
   • Identity card 
   • Birth Certificate or latest extract of Birth Certificate (if any)
   • Citizenship Certificate (if any)
   • Passport or other travel document (if any)
   • One (1) recent passport-sized photograph with a blue background
     
3. PARENTS’ AND SIBLINGS’ DOCUMENTS (COPY)
   • Identity card (if any)
     
4. CHILDREN’S DOCUMENTS (COPY)
   • Birth Certificate (if any)
   • Identity card (if any)

Note: All copies of supporting documents should be made on A4 paper.

1. RM10.00 for application fee
2. RM300.00 to issue Citizenship Certificate (if approved)

All applications regarding citizenship matters must be submitted using the original forms. These forms can be obtained from the NRD (JPN) Headquarters in Putrajaya or any State NRD Headquarters or branch office. For applications concerning the registration of overseas births and the renunciation of citizenship, forms may also be obtained from consular offices abroad.

Citizenship Application Form under Article 15(1) and Article 16 of the Federal Constitution (Form A—obtainable at the NRD counter) has been fully completed.
