Application for Citizenship Status Under Article 15(1) of the Federal Constitution – Wife of a Citizen

Application by mana-mana perempuan that bersuami and suaminya seorang citizen.

1. Bahawa marriagenya masih continuous.
2. That she has resided in the Federation throughout the two years immediately before the date of the application and intends to do so permanently.
3. Bahawa ia berkelakuan baik.
4. That her marriage has been registered under any written law in force in the Federation.

1. Applicant and suami must be present before the Registrar Citizen semasa mengemukakan application.
2. Complete Form A and enclose copies of the required supporting documents.

Applicant and suami is required to be present ke counter Bahagian Citizenship NRD that nearby with membawa original documents together with copies.

1. APPLICANT DOCUMENTS (ORIGINAL AND COPY)
   • Identity Card
   • Birth Certificate or Extract Certificate birth terbaru or Certificate Anak angkat (if any)
   • Marriage document registered under written law in Malaysia
   • Permit Masuk or Extract Butir-Butir dari Register Identity Card applicant
   • Pasport or lain-lain documents perjalanan (if any)
   • Tiga (3) keping photograph terbaru berpassport size berlatar belakangkan warna biru.
     
2. DOCUMENTS SUAMI (ORIGINAL AND COPY)
   • Identity Card
   • Birth Certificate or Extract Birth Certificate terbaru (if any)
   • Certificate Citizen (if any)
   • Pasport or lain-lain documents perjalanan (if any)
   • Satu (1) keping photograph terbaru berpassport size berlatar belakangkan warna biru.
     
3. DOCUMENTS IBU BAPA & ADIK BERADIK (COPY)
   • Identity card (if any)
     
4. DOCUMENTS ANAK ANAK (COPY)
   • Birth Certificate (if any)
   • Identity Card (if any)
     
     Nota: Semua copy fotostat documents-documents sokongan must dibuat on A4-sized paper

1. RM10.00 fees registration application.
2. RM300.00 fees pengeluaran Certificate Citizen (jika is approved).
